As a Case Manager you will work in an addictions based program with adults, age 18 and older, diagnosed with a substance use disorder (SUD) and/or co-occurring mental health and substance disorders (COD), located in Montclair, Essex County NJ.

COPE provides quality behavioral health-care services to the greater Montclair community that are affordable and responsive to individual, family, and community needs. COPE counselors work directly with families in crisis, mainly in the areas of alcohol and drug abuse.

Schedule: Full-time; 3-day shifts; 9am-5:30pm; Evening shifts 11:30-8pm or Saturday 9am-3pm

Competencies: 

Ability to provide  Person Centered, Family-Centered, and Trauma-Informed Care; Understanding and commitment to supporting recovery and resiliency; Ability to assess and interpret information related to population; Knowledge of the symptoms of the disability/disorder and ability to recognize exacerbation of symptoms; General understanding of treatment needs; Knowledge of available treatment resources and appropriate use; Population-specific communication skills; Understanding of Care Coordination; General knowledge of housing subsidies and management of the subsidies.

Responsibilities:

  • Provide a comprehensive case management assessment that addresses life domains such as social support, treatment needs, finances, transportation, legal services and health care;
  • Work within a multi-disciplinary team approach to coordinate services with family, vocational and housing specialists as well as peer supports;
  • Works with both individuals with substance use and co-occurring disorders on obtaining concrete resource needs and will help to coordinate care for individuals who are not eligible for other existing case management services;
  • Work directly with clients who participate in the Contingency Management and Other Strategies for the Treatment of Stimulant Use Disorders Program
  • Maintain strong working relationships with Essex County treatment facilities, acute care centers, resources and stakeholders;
  • Develop and implement individual recovery plans that are person-centered and include strategies for recovery; the plan shall identify priorities, desired outcomes and the strategies and resources to be used in obtaining outcomes based on the case management assessment;
  • Provide training to clients on the recognition of relapse triggers and strategies to deal with avoiding a relapse;
  • Care manager will determine the type and frequency of services desired by person served. The services provided will be responsive to the individual’s needs;
  • Use engagement skills to motivate and encourage individuals to voluntarily access services that would assist them in maintaining a healthy lifestyle;
  • Advocate on behalf of the individual by providing a holistic experience;
  • Document all consumer sessions and outreach rendered thoroughly and timely in the ECR;
  • Participate in multi-disciplinary team meetings and individual supervisions per agency standards;
  • Other Duties as assigned.

Qualifications:

  • Bachelor’s degree in Psychology, Sociology or related field;

  • CADC preferred.

  • Must have 2 years’ experience working with high needs individuals within a behavioral health setting.

  • Valid Driver's License required.
