Position Summary:
This Case Management position will assist and support individuals with intellectual, developmental, or brain injury populations to understand the goals and activities of the person-centered planning process, including assessments. Coordinate the individual’s application, referral, support planning, and monitoring processes in compliance with all applicable policies and procedures.
Essential Functions:
All Case Management activities involving the individual may be inclusive of family members, guardians, and/or other members of a support team, dependent upon legal relationships and individual situations and needs. The Case Manager responsibilities include the following:
Position Type/Expected Hours of Work:
This is a full-time position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm. Occasional evening and weekend work may be required as job duties demand.
Travel:
Travel is required. Travel can be local during the business day, and some out-of-area and overnight travel may be expected.
