Position Summary:

This Case Management position will assist and support individuals with intellectual, developmental, or brain injury populations to understand the goals and activities of the person-centered planning process, including assessments. Coordinate the individual’s application, referral, support planning, and monitoring processes in compliance with all applicable policies and procedures.

Essential Functions:

All Case Management activities involving the individual may be inclusive of family members, guardians, and/or other members of a support team, dependent upon legal relationships and individual situations and needs. The Case Manager responsibilities include the following:

• Engaging in assessment and planning activities including but not limited to:

• Experience with the intellectual and/or developmentally disabled population as well as the brain injury population.
• Assist and support the individual to understand the goals and activities of the person-centered planning process, including assessments.
• Facilitate and schedule participation in person-centered planning and assessments as needed.
• Assist the individual in discovery and exploration discussions, identifying preferences, strengths, goals and dreams, and needs in preparation for person-centered planning process.
• Facilitate the completion of any additional assessments and gathering of documentation as needed, and ensure this information is available for the person-centered planning process.
• Work with the individual to develop agenda(s) and follow up notes for person-centered planning meetings and ensure this information is distributed to members of the support team as approved by the individual.
• Support the individual to lead, and provide assistance as needed, throughout the person- centered planning process.
• Assist the individual and their support team in identifying any needed modifications to the rights and freedoms afforded by the federal Medicaid HCBS requirements based on a documented health and safety need, and ensure that any such modifications are appropriately agreed upon by the individual and their guardian (if they have one) and documented in the ISA/BIP person-centered service plan, including a plan for data collection and review of the modification.

• Responsible for collaborating with service providers within the region to determine service availability and facilitate provider matching. Gathers and sends all necessary information for service authorization including but not limited to, completed needs assessment, person-centered service plan, and accompanying service authorization request, to the State for reporting purposes only.
• Responsible for identifying services available and facilitating provider matching for the Brain Injury Program. Uploads the needs assessment, person-centered plan, and accompanying service authorization request, to the shared electronic record system. The State reviews and approves or suggests modifications to the needs assessment and authorization request or determines that more information is needed. If more information is needed, the Case Manager advises the individual of the additional information that is needed and provides this information to the State.

Position Type/Expected Hours of Work:

This is a full-time position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm. Occasional evening and weekend work may be required as job duties demand.

Travel:

Travel is required. Travel can be local during the business day, and some out-of-area and overnight travel may be expected.

Required Experience:

• Each Case Manager must have knowledge of Vermont’s public service system for developmental disabilities services and brain injury services and at least:

• A Bachelor’s degree in any field and one year of human services related experience such as supporting individuals with disabilities, special education, sociology, vocational rehabilitation and/or employment, protection and advocacy, psychology, substance use, aging, healthcare, social work, or other related field; or
• An Associate’s degree in human services or a closely related field and two years of human services related experience, such as supporting individuals with disabilities, special education, sociology, vocational rehabilitation and/or employment, protection and advocacy, psychology, substance use, aging, healthcare, social work or another related field.
• Must reside in Vermont.
