Working Title: Supports Intensity Scale (SIS) Evaluator

FLSA Status: Exempt

Posting Salary Range: $53,045 - $66,837

Office Location: Remote within Trillium’s Southern Region of our catchment area, which includes North Carolina Counties: Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland.

What We’re Looking For

Trillium Health Resources has a career opening for a Supports Intensity Scale (SIS) Evaluator to join our Member Solutions Team. This position’s primary responsibilities include scheduling, completing and the administration of the Supports Intensity Scale, to include scoring the assessment, preparing a summary report and completion of other related reports. In addition, the SIS Evaluator may conduct other assessments/evaluations as requested.

On a typical day, you might:

  • Schedule and administer the Supports Intensity Scale (SIS) assessments as assigned including reassessments and amendments. 
     
  • Develop a written report of the results of completed assessments, including conclusions, support needs index scores and supplemental questions assessment in a timely manner.
     
  • Assist with provision of education/training as needed to promote positive outcomes of SIS initiative.
     
  • Ensure responsiveness to feedback/questions/concerns from individuals, families and providers related to the SIS.
     
  • Collaborate with other members of the SIS team to ensure consistency between interviewers.

Qualifications: 

Education:

Bachelor’s degree is required.

 

Preferred Education:

A degree in Special Education, Psychology, Social Work or closely related Human Services area

 

Experience:

Bachelor’s degree and two (2) years of post-degree progressive experience providing similar services to individuals with Intellectual and/or other Developmental Disabilities.

OR

Equivalent combination of education/experience.

 

License/Certification:

Must have a valid driver’s license

Must be certified to administer the SIS, which may be attained after commencing employment with Trillium.

 

Location:

  • Remote within Trillium’s Southern Region of our catchment area, which includes North Carolina Counties: Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland.
  • Must reside in NC to be considered for remote status.
  • Must be able to travel to a Trillium office location and within catchment as required.
