Job description

General Summary: The Community Care Assistant responsibilities will be to contact prospective patients via telephone from information collected from Marketing events, while also assisting with targeted campaigns with patients. This position will also assist with identifying patients in need of benefit review, plan changes, and PCP changes by directing to the sales team to schedule for CHA team. The Community Care Assistant is responsible for answering incoming calls and making outbound calls to and from Managed Care enrollment prospects, patients, families, and other callers for all Gonzaba Medical Group (GMG) clinics.

Supervisory Responsibilities: This position has no supervisory responsibilities.

  • Duties: Performs by making outbound calls from leads gathered by Marketing engagements.
  • Accepts inbound calls from prospects and/or patients regarding Medicare and/or Medicare Advantage plans.
  • Responsible for making outbound calls related to Retention efforts for our Marketing department.
  • Answers inbound and performs outbound calls promptly in a courteous, professional, and mature manner.
  • Represents a positive image of GMG, providing exemplary customer service.
  • Communicate daily with Community Care Connect Manager.
  • Able to work efficiently over the phone and is comfortable under pressure.
  • Represents a positive image of GMG, providing exemplary customer service.
  • Detail-oriented mindset, capable of maintaining accuracy and consistency in evaluations.
  • Other duties as assigned.
