Job Summary:

The Clinical Nurse (RN) plays a vital role in ensuring the seamless
coordination of mental health care for clients. This position requires
a licensed Registered Nurse (RN) with strong clinical expertise,
problem-solving abilities, and excellent follow-up skills in a mental
health setting. The ideal candidate will demonstrate patience and
empathy when working with clients, fostering trust and building
supportive relationships.

This role focuses on care coordination, ensuring clients receive
comprehensive, timely, and effective treatment through wraparound
services, including therapy, day rehabilitation, and medication
management. The Clinical Nurse (RN) will work in collaboration with
therapists, psychiatrists, social workers, and primary care providers
to integrate mental health and general healthcare into a cohesive
plan. The position includes daily staff communication, treatment plan
follow-ups, documentation, advocacy, and continuity of care—all
essential for client success.

This is a full-time role with virtual work flexibility, allowing for
remote coordination while maintaining in-person support when
necessary.

Care Coordination in Mental Health

Mental health care coordination is the structured effort to ensure
clients receive high-quality, holistic care tailored to their unique
needs. It involves streamlining communication among healthcare
providers, minimizing treatment gaps, and ensuring smooth transitions

Key Components of Mental Health Care Coordination:

Interdisciplinary Collaboration: Align treatment plans across

Advocacy & Education: Empower clients and families with information

support.

Clinical Documentation: Accurately record patient progress in
electronic health records (EHR) systems while ensuring compliance with
healthcare regulations.

Problem-Solving & Follow-Up: Address challenges in client care,
proactively identifying solutions and ensuring timely responses.

Resource Navigation: Connect clients with essential wraparound support
services, including housing assistance and employment programs.

Transition Management: Guide clients through changes in care settings
while maintaining continuity of care.

Advocacy & Education: Educate clients and families about mental health
resources, helping them navigate the healthcare system effectively.

Qualifications & Skills:
