ABOUT THE ROLE

JOB SUMMARY:

The Clinical Coordinator (CC) is a Homecare Homebase (HCHB) superuser responsible for the timely completion of assigned daily workflow, maintenance of back-office functions, medical coding, and support of the Area Vice President of Operations (AVP), the Executive Director of Clinical Operations (EDCO), the Director of Clinical Operations (DCO), and the direct patient care team. The CC will communicate identified deficiencies to the AVP or designee to prioritize follow-up as applicable. The CC will conduct internal performance improvement audits and compile quality assurance performance improvement data for all assigned locations. CC will collaborate with local leadership, the Compliance team, and the Clinical Educator to ensure regulatory compliance and identification of educational needs for all assigned locations.

ROLE REQUIREMENTS

  • This is a remote position but must live in one of the following states: Arizona, Utah, Colorado, or Georgia
  • Schedule is Monday-Friday 8:00am - 5:00pm MST.
  • Willing to travel up to 15% of the time


REQUIRED EDUCATION/EXPERIENCE:

  • Bachelor’s degree
  • Active license as a Registered Nurse in the state in which we do business, AZ, UT, CO, GA
  • Two (2) years of clinical experience in hospice/home health
  • Must possess a valid driver’s license, documentation of valid vehicle insurance, and reliable transportation.


SKILLS/ABILITIES/KNOWLEDGE:

  • Excellent observation skills, along with the ability to make sound decisions based on nursing knowledge (evidence, theories, ways/patterns of knowing), other disciplinary knowledge, critical thinking, and clinical reasoning.
  • Thorough knowledge of managed care principles, hospice care delivery guidelines, laws, and regulations.
  • Appropriate communication skills, showing respect and compassion to the learner.
  • Proficient in the Microsoft Office suite of products and related computer skills.
  • Proficient in ICD-10 coding.
  • Ability to document according to standards of practice and to audit the documentation of others utilizing resources and tools accurately and efficiently provided.
  • Demonstrated record of accomplishment in developing and managing successful teaching strategies, improving the quality of patient care and compliance outcomes, and successfully following various performance improvement initiatives and plans of correction.
  • Consistently produce work that is detail-oriented and accurate.
  • Possess a professional and resourceful style, can work independently and as a team player, takes initiative, and manages multiple tasks and projects at the same time.
  • Consistently produces work that is detail-oriented and accurate.
  • Demonstrate acceptable interpersonal, oral, and written communication to maintain a positive environment.
