The Home Health Aide will provide routine personal care and assistance that enables patients to remain safely in their homes. This role is vital in supporting the patient’s health, comfort, and dignity through day-to-day care and monitoring.
• Monitor and record patient condition by observing physical and mental health, intake and output, and activity.
• Support patients by providing housekeeping, laundry services, grocery shopping, meal preparation, and running errands.
• Assist patients with personal services such as bathing, dressing, grooming, and toileting.
• Teach patients to care for themselves using mobility aids, adaptive utensils, and personal hygiene tools.
• Educate family members on how to safely lift, turn, and reposition patients.
• Advise patients and families on cleanliness, nutrition, and maintaining a healthy home environment.
• Maintain accurate documentation in the patient journal and notify nursing supervisor of any unusual conditions or changes.
• Ensure a safe, secure, and sanitary environment by following standard infection control procedures and dietary requirements.
• Adhere to all professional, agency, state, and federal standards and regulations.
• Enhance the agency’s reputation by taking initiative, responding to special requests, and contributing positively to the patient care experience.
• High school diploma or equivalent.
• Current and valid Home Health Aide (HHA) Certificate (California-issued).
• Must be at least 18 years old.
• Reliable transportation.
• Able to pass a criminal background check.
• Able to provide professional references.
• Ability to follow written and verbal instructions.
• Demonstrates good personal hygiene and professionalism.
• Ability to work independently with limited supervision.
• Capable of performing light housekeeping duties.
• In-home care setting; may include private residences, apartments, or hotel accommodations.
• Must be able to travel locally within assigned service areas.
