Key responsibilities - 
  • Identify eligible patients for palliative services in collaboration with housecalls primary care team. 
  • Conduct patient and caregiver education on the goals and scope of palliative care.
  • Perform intake assessments and support appropriate documentation for transitions.
  • Collaborate with clinical teams, case managers, and community providers.
  • Ensure timely and appropriate transition to hospice when indicated.
  • Maintain accurate and timely documentation in EMRs and referral systems.
  • Participate in interdisciplinary team meetings and case conferences.
 
 
Qualifications -
 
  1. High emotional IQ,
  2. 5+ years of experience in the healthcare field
  3. 2+ years of experience in palliative, hospice, or transitional care
  4. Knowledge of end-of-life care, chronic illness trajectories, and advance care planning
 
Skills- 
  1. Excellent communication and interpersonal skills
  2. Ability to manage sensitive conversations with patients and families
  3. Strong organizational and time management abilities
  4. Collaborative mindset and team orientation
  5. Cultural competence and sensitivity
