Primary Work Location:
Fairmont Respite is a tiny home community located near Fairmont Hospital. Cardea Health provides clinical services at Fairmont Respite, a 34 bed tiny home community that services individuals with clinical needs and provides housing navigation services. Cardea Health partners with Alameda County for the Homeless agency and the housing agency Five Keys to provided tailored services to unhoused invididuals working to transition to permanent housing.
Job Summary:
The Associate Program Manager- Fairmont Tiny Homes will assist with direct personal and program activities for Cardea Health Transitional Housing Programs, which is a large recuperative care program serving people who have experienced homelessness. The Associate Program Manager is accountable for assisting with managing facilities, clinical personnel, housekeeping/caregiving functions, food service, transportation, and housing navigation and all care coordination needs at our Fairmont Tiny Home location. The Associate Program Manager will work with the nursing team to manage the flow of clients through the program, from admission to discharge. In addition, the Associate Program Manager will carry out care coordination duties at the site in conjunction with the site’s nursing team.

Proficiency in Microsoft Office Suite is required

Minimum Required Skills & Qualifications:
Bachelor’s degree or equivalent professional experience
At least two years of related experience required
Proven experience as a team leader and operations professional
Experience working with people experiencing homelessness preferred
Experience working in healthcare preferred
Program Management Job Duties and Responsibilities:
Assist the Program Director of Transitional Housing and Director of Nursing for Transitional Housing with leading a team of dedicated clinical and non-clinical professionals
Oversee facilities and operations at a site of residential tiny home units
Manage personnel and operations related to
Clinical staff
Case management
Housekeeping
Caregiving
Food service
Transportation
Additional Responsibilities:
Troubleshoot issues as indicated
Escalate challenges when appropriate
Manage client experience in the program from referral to discharge
Assist with data reporting as required for assessment of program performance
Maximize program efficacy in securing service connections and permanent housing for clients
Ensure that clients move through program in accordance with program parameters
Fill all beds promptly
Care Coordinator Job Duties & Responsibilities:
Maintain a calendar of upcoming resident medical and behavioral health appointments. Communicate upcoming appointments to clients, clinical staff, and transportation services
Assist clinical staff and residents in the scheduling of client medical and behavioral health appointments
Communicate with local pharmacies regarding resident prescriptions. At the direction of nursing staff request medication refills, deliveries, and transfers
Request relevant medical records of residents from hospitals and community clinics. Transmit medical records and other authorized requested documentation to partner clinics to facilitate care coordination
Interface with resident healthcare coverage plans to avoid interruption in services and facilitate care
Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls
Ensure adherence to infection control standards on site among staff and residents
Answer calls to nursing station and respond independently to when appropriate
Assist nursing staff with light clerical duties including filing and making copies
Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA)
Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect
Other duties and responsibilities as required
Physical Demands:
Sit, walk, and stand frequently.
Occasionally lift and/or move up to 20 pounds
Work Environment:
This position will require a daily on-site presence at a supportive housing facility
Compensation & Benefits:
Competitive Salary: ($72,000-$90,000)
