Perry Health’s mission is to deliver high-quality continuous care to chronically ill patients. We started with Diabetes and serving the Medicare population and those populations that don’t typically have access to high quality care between in-person appointments. Over 130M Americans suffer from chronic disease and that number is expected to reach 170M by 2030. What is even more staggering is that 85% of seniors suffer from a chronic disease. The problem as we see it is that in person care is not adequate for patients living with chronic disease, leading to poor outcomes and increased cost. This is why we created Perry.
Perry’s first product is a cellular connected blood glucose monitor supported by a first-of-its-kind continuous Electronic Health Record (EHR) platform and a team of world-class clinicians and coaches. Our users receive 24/7 access to our service at little to no out-of-pocket cost to them. We provide continuous monitoring of blood glucose levels and proactively reach out to our users when things aren’t trending the right way. We also support our members through regular clinician check-ins and nutritional support. Our program creates real outcomes for our members and thousands of dollars in savings per patient to the healthcare system. And we’re just getting started.
