This position is remote but requires in home visits across region 4. Travel may vary and the region covers Kentucky, Tennessee, North and South Carolina, Mississippi, Alabama, Georgia, and Florida. Applicants can be from any of these states since the position is remote.

Position Summary:
Working under a grant from the Office of Refugee Resettlement (ORR), NYAP’s Post Release Services (PRS) Intensive Case Manager Level 3 provides post release services focused on stabilizing the family unit utilizing trauma-informed and intensive case management. The PRS Intensive Case Manager Level 3 will additionally connect with community-based services to address concerns that require long-term care. This position is required to travel to the initial in-home assessment followed by weekly in-person contacts for the first 45 to 60 days, then monthly in-person or virtual contacts for 6 months, depending on the needs of the family.
