We are currently seeking a dedicated and detail-oriented Case Coordinator to join our dynamic team. Responsibilities:

  1. Case Management:

    • Coordinate and manage the overall case management process, ensuring the delivery of quality and timely healthcare services.
    • Collaborate with healthcare professionals to develop and implement individualized care plans.
  2. Patient Advocacy:

    • Advocate for patients' needs and rights, ensuring they receive comprehensive and compassionate care.
    • Serve as a liaison between patients, families, and healthcare providers.
  3. Coordination of Care:

    • Facilitate communication and coordination among members of the healthcare team.
    • Organize and oversee appointments, tests, and consultations to ensure continuity of care.
  4. Documentation:

    • Maintain accurate and up-to-date patient records, including assessments, care plans, and relevant documentation.
    • Ensure compliance with regulatory standards and organizational policies.
  5. Resource Utilization:

    • Optimize the use of healthcare resources by coordinating services efficiently and avoiding duplication.
    • Assist in identifying appropriate community resources and support services.
  6. Patient Education:

    • Provide information and education to patients and their families about their medical conditions, treatment plans, and available resources.
    • Reinforce healthcare provider instructions and recommendations.
  7. Quality Improvement:

    • Contribute to quality improvement initiatives by participating in case reviews and evaluations.
    • Implement changes to enhance the efficiency and effectiveness of case coordination.
  8. Crisis Intervention:

    • Respond to crisis situations and coordinate appropriate interventions in collaboration with the healthcare team.
    • Work to resolve issues affecting the continuity of patient care.

Qualifications:

  • Bachelor's degree in Healthcare Administration, Nursing, Social Work, or a related field.
  • Proven experience in case coordination or a similar healthcare role.
  • Knowledge of healthcare regulations and compliance standards.
  • Excellent organizational and multitasking abilities.
  • Strong communication and interpersonal skills.
  • Graduate of Bachelor's Degree (preferably related to medical).
  • With at least 2 years of work experience as a Case Coordinator or Patient Relations Officer in a healthcare facility such as Homecare, Hospital, Medical Center, or Clinic.
  • With Good Communication Skills in Arabic & English.
  • Immediately Available.
