YOUR ROLE
As the Chief Clinical Officer with Evergreen Nephrology, you lead, influence, and ensure proactive clinical care and optimal clinical quality for our patients. The Chief Clinical Officer will serve as a key member of the clinical management team and report directly to the Chief Medical Officer (CMO) to drive Evergreen Nephrology’s clinical success.
The Chief Clinical Officer will work closely with the CMO and have the responsibility to provide clinical and strategic direction and oversight of Evergreen’s clinical model and initiatives to ensure high quality patient care while meeting financial, operational, and clinical quality metrics.
PRIMARY FUNCTIONS
- Provide clinical oversight and supervision to Evergreen Advanced Practice Providers caring for aligned patients
- Serve as Medical Director for the Evergreen Aligned (EGA) patient population focusing on value-based initiatives to improve quality while reducing cost of care, clinical innovation, performance improvement, clinical transformation.
- Ensure a comprehensive medical and behavioral approach to clinical program design across multiple disease states including diabetes, hypertension and other chronic diseases that lead to kidney disease progression, in collaboration with the CMO, SVP of Clinical Operations and COO
- Oversee operational effectiveness and quality performance of the Quality Incentive Program (QPIP) partner practices to ensure optimal performance in achieving the contracted metrics and accomplishing program goals.
- Assist the CMO in the development kidney care clinical programs focused on delaying progression of CKD, increasing adoption of home dialysis and kidney transplantation, and ensuring proper goal setting and shared decision-making regarding potential kidney replacement therapies.
- Assist the CMO in equipping and overseeing JV Partner Medical Directors as they lead their JV practice towards the realization of quality, cost-effective health care services that accomplish Evergreen’s value based care goals.
- Participate in the Evergreen Physician Leadership Council meetings as assigned.

- You have completed an accredited residency in Family or Internal Medicine (fellowship training in nephrology or another specialty is optional).
- You hold an unrestricted medical doctor state license and are board certified by an American Board of Medical Specialties (ABMS) approved certifying agency.
- You have 5+ years of clinical practice experience
- You have a minimum of ten (10) years leading people and organizations to success.
- You have proven experience leading significant improvements in clinical quality and outcomes for care delivery and/or managed care organizations
- You have strong comprehension of clinical standards of care and are focused on quality of life for your patients
- Bonus points: You have a master’s degree or higher related to quality, public health, administration, health services research or a related field
- You may also have experience or be certified in one or more of the following areas:
- Specialist in Clinical Hypertension
- Hospice and Palliative Care Medicine
- Kidney Transplantation
- Intermediate skills with MS Office Suite of products including Outlook and Teams
- You have the ability to work effectively in a primarily remote environment
- At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wired to the house internet (Cable, Fiber, or DSL) and hardwired to the internet device is recommended
- Evergreen will provide Remote or Hybrid Home/Office employees with telephony applications and equipment to meet the business requirements for their position/job
- Team Members must work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
Pay range for this role is $315,000 to $340,000, with exact pay determined based on experience, education, demand for role, geographic location and other role-specific criteria.
