Job description

UPMC Workpartners is hiring a full-time Clinical Prevention Specialist Nurse. This position will provide data driven clinical decision support service to high risk individuals and families enrolled in the HCMS Group KnovaSolutions program. This position provides primarily telephonic support from a home office as part of the HCMS Clinical Prevention Service comprised of nurse, pharmacist, and medical team.

Responsibilities:

  • Additional Responsibilities for Quality Assurance Team: Perform call monitoring for the clinical team. Complete audits and competency review, provide regular feedback to the Clinical Leadership team. Develop and monitor orientation and mentor program for new clinical staff. Document coaching sessions and track monthly progress. Suggest and facilitate skill trainings to improve service delivery. Provide guidance to staff with questions related to service delivery. Facilitate small group discussions for staff to review KnovaSolutions members and plan of care. Support clinical staff with challenging calls from members and escalate as needed.
  • Maintains patient confidentiality, adhering to HCMS HIPPA & Privacy Policies & Guidelines. Accurately prepares high-quality professional written consultation and service documentation.Initiates, develops, and maintains effective working relationships with clinical prevention service team members.
  • Research clients previously diagnosed medical conditions to better understand the diagnosis, interaction with other diagnoses, and treatment options. Discuss patients' desired treatment outcomes, wellness options, and lifestyle. Provide guidance in navigating multiple specialists, diagnoses, and pharmaceutical treatments.
  • Provide clinical prevention decision support to individuals and families over the telephone and via email. Maintains quality documentation of service delivery within the Service Management Application.
  • Applies critical thinking, innovative problem solving and clinical support skills to promote quality decision making by KnovaSolutions Members as Your Health. Your Decisions.
  • The Clinical Prevention Specialist uses pro-active cognitive listening and assessment skills to provide service to high risk people enrolled in the KnovaSolutions Clinical Prevention service. Adherence to the HCMS Home Sourcing and Non-traditional Workplace Policy.
  • Graduate of an accredited school of nursing
  • Registered Nurse with current licensure required. Compact RN strongly preferred.
  • Bachelor's of Science in Nursing or related field
  • Minimum 5 years nursing experience
  • Bilingual Spanish is a bonus.
  • Excellent verbal communication and with individuals and groups Work independently with excellent organization, and time management skills

Licensure, Certifications, and Clearances:

  • Registered Nurse (RN)
  • Act 34
  • Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
