About the Role

We’re looking for a Care Navigator to join our existing care team who has prior experience coordinating/navigating care for patients, caregivers, and their loved ones - bonus points if you speak Spanish and you’ve worked with a patient population who experience cognitive challenges. This role requires genuine empathy, patience, exceptional time management and communication skills.

Responsibilities

  • Answer inbound calls, emails and text messages from patients, caregivers, families, external providers and other facilities
  • Schedule and maintain contact with patients via phone or email
  • Explain health insurance benefits and eligibility to patients, process copays and coinsurance as needed
  • Under clinical supervision, ask screening questions and document patient safety, behavior, function, medications, care needs, and advanced care plans
  • Under clinical supervision, ask screening questions and document caregiver issues such as burden, depression, and poor coping
  • Report screening to the clinical team (nurse, social worker, doctor) who will provide direction in implementing a care plan with the Care Navigator, patient, caregiver, and other health care and community service providers involved in the patient’s care
  • Send educational materials and information about care with Isaac and other local community based service options as indicated in the patient’s care plan
  • Follow up after all appointments to provide next steps and coordinate care as documented by the provider
  • Request external medical records, labs and imaging results
  • Promote a compassionate environment by providing emotional and psychological support to patients, caregivers and families
  • Provide comfort by anticipating anxieties, providing supportive resources, and answering questions within the scope of practice
  • Document patient care and instruction in EHR
  • Coordinate patient care with external clinics, providers and facilities as documented in the EHR by the provider
  • Escalate patient concerns to the clinical team to maintain an appropriate scope of practice for this role
  • Maintain participant’s privacy according to HIPAA regulations
