Summary of Job Duties:
This is a temporary position not benefits eligible.

The PRN Care Management Assistant (CMA) position will support the Care Management Department and teams with clerical support for the discharge planning and utilization review process of patients at UAMS. The discharge planning process starts with admission. The PRN CMA will communicate with post-acute facilities, patients, families and other UAMS team members to ensure that services are arranged as ordered.

Extra-Help positions are required to work two (2) shifts a month with a maximum of 1000 hours in a 12 month floating period. Concurrent employees work one (1) shift a month with a maximum of 1400 hours per year in a 12 month floating period.

Qualifications:

Minimum Qualifications:

  • High School Diploma/GED Plus three (3) years of CMA, direct patient care, medical records, customer service or related experience OR Associates Degree + One (1) year of admin experience/documentation experience

Preferred Qualifications:

  • Bachelor's Degree
  • Registration and appointment scheduling experience with EPIC

Preferred special knowledge, abilities and skills:

  • Medical Terminology; experience working with DME companies or other post-acute care organizations.

Special knowledge, abilities and skills:

  • Computer/keyboard skills; telephone etiquette; general knowledge of office equipment.

Physical Requirements:

Employees in this role must be fitted for, capable of donning and performing routine tasks in personal protective equipment to include N95 masks, Powered Air Purifying Respirators (PAPR) or Controlled Air Purifying Respirators (CAPR).


Additional Information:

Responsibilities:

  • Works collaboratively with the RN Care Manager and Social Work team members focused on care of the patient and family to facilitate timely discharges and support a smooth transition to the next level of care for all patients.
  • Communicates the coordinated referrals to post-discharge providers, including home health, home and inpatient hospice, Long Term Acute Care facilities, Skilled Nursing Facilities, Assisted Living facilities, transportation and others as needed for a safe discharge plan. Gathers and provides all relevant information needed by facilities to provide for care transitions, following policy with regards to release of information.
  • Conducts follow-up calls with providers to ensure receipt of needed information and expected timing of acceptance.  
  • Arranges discharge appointments for patients following up at UAMS or other community providers.
  • Ensures that the appointments are listed as part of the patients’ discharge education. 
  • Demonstrates a working knowledge of payor benefits (Medicare, Medicaid as well as managed care and commercial payors).
  • Completes required regulatory documentation for patient admissions with the guidance of Care Management team (examples include, but are not limited to, MOON notices, Medicare Important Messages etc.).
  • Interacts with, and assists, the public in a professional and friendly manner as needed.
  • Demonstrates effective communication skills; communicates accurate and complete information; maintains strict confidentiality when necessary.
  • Demonstrates positive working relationships with co-workers, management team, and ancillary departments; follows the Core Concepts of Patient and Family Centered Care and the Basic Code of Conduct Guidelines.
  • Assists internal and external stakeholders, as needed, with exceptional customer service.
  • May perform other duties as assigned.

PRN employees are typically required to meet the following or equivalent criteria:

  • Work the equivalent of 24 hours on a regularly scheduled basis (monthly).   The 24 hours is typically 3-8 hour shifts, but the shifts could change based on the needs of the department.
  • These shifts will be scheduled at a time consistent with departmental staffing needs, and mutually agreed upon by the Manager and PRN Staff member.  
  • Work 1-2 designated UAMS holidays (excluding Birthdays) and four weekend shifts a year (1 per quarter) based upon the needs of the department.
  • Attend HR Orientation and complete all HR new hire requirements.  Department orientation must be scheduled and completed within 90 days of hire.
  • PRN staff must comply with all departmental scheduling procedures, attendance policies, and clinical performance standards.
  • PRN staff are only allowed to work 1000 hours in a 12 month rolling period, which begins at orientation and includes any educational hours.
  • PRN staff assignments/shifts are subject to cancellation and the PRN employee will be notified by the CSM or designee at least one hour prior to the beginning of the shift. 
  • PRN staff are not eligible to receive benefits, but may contribute to pre-tax supplemental Retirement.
