Summary of Job Duties:

The Primary Care Social Worker reports to the Behavioral Health Program Manager and may also receive direction from licensed social workers on the patient’s Care Team. In addition to caring for patients, the Social Worker provides supportive services to family and care team members throughout the patient’s assessment and treatment.

• Bachelor's of Social Work from an accredited program.
• 2 years of experience in social work, counseling, education or a related field - preferred.
• Must have a current Arkansas Social Work License-LSW

• Duties may include, but are not limited to, meeting with patients, orienting them to the practice, conducting a needs assessment, providing referrals to community services, faxing confidential patient records to Behavioral Health providers, and speaking with patient family members.
• Patient contact may include reviewing patient’s self-management goals, reviewing documents, like advanced directives, with patients for their signature, confirming patient family contact information or collecting other patient information necessary for care planning.
