Job Summary And Qualifications**

The Performance Improvement Specialist supports providers within the VCP network to include monitoring progress towards network goals, quality care gap management, practice transformation to improve process flows, transitions of care for identified populations, and care coordination to drive quality and cost outcomes for the attributed populations.

  • You will assess assigned practice workflows and processes that are in place to close gaps in care, make outreach to patients for transitions of care, and the ability to perform care coordination activities and then assist with improving those processes to achieve the desired results within the network.
  • Identify attributed patients who benefit from care coordination and work with practice to manage the high risk, high cost, and high utilizers.
  • Assist with referring to appropriate ancillary services such as payer specific CM/DM programs, BH resources, pharmacist support, and other resources as identified.
  • Assist assigned practices with managing their populations that can include, but not limited to, specific chronic diseases, high ER utilizers, medication compliance, and other identified areas of focus.
  • Assist and perform transitions of care on the identified populations.
  • Meet with practices both virtually and onsite to engage practice with network and develop a plan to work towards the identified network goals.
  • Monitor practice progress on an ongoing basis, which may include analysis of data and identifying opportunities for improvement.
  • Identifies problematic issues, determines reasons and actions taken to resolve, and escalates appropriately

What qualifications you will need:

  • BA or LPN license required
  • Healthcare experience preferred.
  • Knowledge of primary care practice process preferred.
  • Experience with evaluating and monitoring data around quality metrics preferred.
  • Proficiency with MS Word, Excel, and Power Point required. Experience with SharePoint preferred.
  • Establish and maintain long-term customer relationships, building trust and respect by consistently meeting and exceeding expectations
  • Effectively manage the project controls of complex processes
  • Clear and confident communicator internally and with members/external partners; Confident public speaking abilities
  • Passionate about improving the healthcare system
  • Unyielding attention to detail
  • Ability to work REMOTE if local to the Richmond Metro area and also able to travel within the Richmond Metro/Tri-Cities region
