  • Serve as the point of contact for external stakeholders regarding ACT, CST, and TMS services.

What We’re Looking For

Trillium Health Resources has a career opening for a Practice Management Consultant in Adult Behavioral Health to join our team. This position’s primary responsibilities include supporting, monitoring and ensuring the quality of Trillium’s contracted adult behavioral health providers, including ACT, CST and TMS. These recovery-oriented services are intended to support individuals with severe and persistent mental illness (SPMI) in maintaining stability in the communities of their choosing and promote opportunities for community inclusion and engagement. Focusing on these services will support Trillium in meeting the requirements of the Department of Justice Settlement Agreement for individuals who are part of Transition to Community Living (TCL). The ideal candidate would have extensive professional experience in monitoring and conducting audits of adult behavioral health services to determine compliance and quality of care. On a typical day, you might:

  • Maintain knowledge of applicable policies for adult BH services, including ACT, CST and TMS.

 

  • Participate in Department of Justice (DOJ) reviews and maintain knowledge of applicable requirements.

 

  • Identify barriers within ACT, CST and TMS services to meeting requirements of DOJ Settlement Agreement. Develop, implement and monitor initiatives to overcome barriers.

 

  • Maintain a fluid list of ACT, CST and TMS teams to include but not limited to: team and provider leadership contact information, counties covered, and capacity.

 

  • Represent Trillium in the Provider Fidelity Review process. Maintain information related to fidelity reviews including dates of reviews, provider outcomes and scores. Track data and identify systemic needs to improve fidelity. Develop plans to support Provider Network on areas of deficiency.

 

  • Serve as the point of contact for external stakeholders as it relates to ACT, CST and TMS services.

 

  • Schedule, coordinate, and perform ACT, CST and TMS provider monitoring to ensure compliance with rules, regulations, service definition requirements and quality standards of practice.

 

  • Implement and maintain inter-rater reliability when conducting provider monitoring.

Qualifications: 

Education/Experience:

Must meet Mental Health QP status, to include professional experience in North Carolina Medicaid funded service:

  1. Fully licensed by the NC governing board regulating Human Services professions; OR

Fully licensed RN who is licensed to practice in the state of NC by the NC Board of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served. OR 

  1. A graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, pre or post-graduate degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has one year of full-time, pre or postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR
  2. A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has two years of full-time, pre or post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR
  1. A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has four years of full-time, pre or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling. 

Preferred Education/Experience: 
Prefer Human Services degree.
Experience with Quality Management processes and/or tools commonly used for monitoring and auditing purposes.

Experience working on a team or as a team lead for Assertive Community Treatment, Community Support Team, and/or Transition Management Services (ACT, CST, and/or TMS).

License/Certification:
May be accepted as noted above. 
 Must have a valid driver’s license

Location:
Remote within Trillium’s Catchment area of North Carolina.
