The Job
Contribute to the care and well-being of SCAN members, empowering them to age in place by collaboratively assessing, planning, coordinating, and monitoring their holistic needs. Focus on addressing members’ care coordination needs, processing referrals and services, and supporting navigation of the healthcare system.
You Will
Perform assessments and screenings to determine and provide appropriate interventions and referrals.
Act as a triage and starting point of contact with members, providers, and medical groups for administration of various benefits/programs and to troubleshoot access issues for dually enrolled Medicare/Medi-Cal SCAN beneficiaries and other SCAN members as needed.
Use motivational interviewing techniques to understand and accurately address member goals and concerns.
Provide basic care management through coordination of care utilizing member benefits and community resources, addressing and resolving access to care issues to support better health outcomes and a positive member experience.
Complete timely and accurate documentation, such as assessments, care plans, service plans, and case notes, in multiple computer systems and ensure corresponding documentation is submitted within established timeframes and following department desktop procedures and/or training materials.
Demonstrate critical thinking skills to resolve issues with members, providers, and vendors.
Coordinate care and/or services through collaboration with county offices and governmental agencies (e.g. Social Security Administration, Department of Public Social Services, LA County Department of Mental Health) as needed.
Exhibit professionalism and empathy in all interactions with members and provider partners.
Utilize department desktop procedures, workflows, job aids and training material.
Adhere to all quality, compliance, and regulatory standards, and supports the achievement of SCAN’s Vision and Goals.

Your Qualifications
High School Diploma required. Some college preferred.
BILINGUAL Strong Preferred - In English and Spanish or Mandarin or Cantonese or Tagalog or Korean or Vietnamese. A test will be administered to assess skills.
1 or more years of experience working in healthcare/customer service.
Technical expertise - Basic technical skills for functional area
Problem Solving - Basic problem-solving skills
Communication - Good communication and interpersonal skills
Use motivational interviewing techniques to understand and accurately address member concerns.
Ability to work in an environment where continuous coaching and feedback is the standard practice.
Basic interpersonal skills, including excellent written and verbal communication skills.

Ability to collaborate effectively within a multidisciplinary team.
Ability to appropriately maintain confidentiality.
General understanding of CMS and DHCS regulations.
General knowledge of medical terminology and abbreviations.
General knowledge of local community resources for seniors.
