Job description

Introduction

The Department of Health Services (DHS) Division of Quality Assurance (DQA), Bureau of Assisted Living (BAL) is recruiting for an Assisted Living Surveyor in the Northeastern Regional Office. DQA is responsible for protecting and promoting the health, safety, and welfare of residents living in and being cared for in regulated health care facilities.

This Position

  • Works remotely and must have access to both a private workspace and secure high-speed internet.
  • Will be required to report to the Northeastern Regional Office Headquarters located at 200 North Jefferson Street in Green Bay at least once per month.
  • Requires approximately 50%-75% travel throughout the northeastern region of Wisconsin but may also include other regional travel as needed. Overnight stays may be required. Click this link to see a map of the regions.

Position Summary

As An Assisted Living Surveyor, You Will Evaluate, License, And Monitor Assisted Living Facilities Including Community-based Residential Facilities, Residential Care Apartment Complexes And Adult Family Homes. You’ll Need To Determine Compliance With DHS Regulations Which Protect And Promote The Health, Safety And Welfare Of The Persons Being Served. You Will Also Ensure Compliance With Standards And Rules In

  • admissions, transfers, and discharges;
  • resident rights;
  • staffing;
  • quality of care assessment;
  • quality of life assessment;
  • resident individual service plans;
  • social and psychological services;
  • provision of medical services; and
  • therapeutic activity programming.

In Addition, You Will

  • review compliance with physical environment requirements and life safety,
  • recommend and participate in enforcement,
  • investigate complaints against licensed and unlicensed programs,
  • conduct verification and surveillance visits,
  • provide technical assistance to individuals or groups involved in the provision of assisted living residential care.

Salary Information

The starting pay is between $29.35 to $34.80 per hour (approximately $61,000 to $72,300 per year), depending on experience, plus excellent benefits. A 12-month probationary period is required.

Pay for current or former state employees is set based upon the rules that apply to compensation for the applicable transaction. This position is classified as Health Services Specialist and is in pay schedule and range 12/63.

Job Details

  • All applicants who may be appointed will be required to allow DHS to conduct a background check to determine whether the circumstances of any conviction may be related to the job being filled.
  • For positions that allow remote working flexibility, working outside of Wisconsin or in bordering cities will not be allowed except in extremely limited circumstances due to the need to report to an office location in Wisconsin on short notice.
  • DHS does not sponsor work visas for this position, thus, in compliance with federal law, selected candidates will be required to verify eligibility to work in the United States by completing the required I-9 form upon hire.
  • Must meet the requirements of the State of Wisconsin Fleet Vehicle Policy which requires a valid driver’s license, minimum two years of driving experience and must be 18 years of age or older. Additionally, the driving record must not reflect the following conditions: three or more moving violations and/or at fault accidents in the past two years, an OWI or DUI violation within the past 12 months, or suspension or revocation of the driver's license.
  • All appointees may be subject to DHS' flu-shot policy.

Qualifications

Minimally qualified applicants will have all of the following:

  • Experience receiving, triaging, or responding to complaints.
  • Experience interpreting and applying assisted living statutes, administrative rules, or other long-term care regulations.
  • Experience using computer programs such as Microsoft Office Suite for data entry, document creation, and report production.

Well-qualified applicants will also have one or more of the following:

  • Experience working with individuals with advanced age, intellectual disabilities, mental illness, or dementia in a healthcare capacity.
  • Experience being involved in the healthcare survey or regulatory review process (e.g., preparing for a survey, conducting a survey or conducting a complaint investigation, reviewing investigation reports, implementing action plans, etc.).
  • Experience conducting investigations (e.g., conducting interviews, reviewing records, gathering observations, writing reports based on findings, etc.).
