What We’re Looking For

Trillium Health Resources has a career opening for an RCT Regional Manager to join our Regional Care Team. This position oversees a skilled, integrated team of professionals who provide localized support within a managed care environment for members living with moderate to severe mental health and/or, substance use conditions and are eligible for plan-based Tailored Care Management with Trillium Health Resources. On a typical day, you might:

  • Direct Supervision of the RCT Supervisors of Care Management and clinical activities performed by the Regional Care Team Provides on –going advocacy and education regarding MH/SU conditions and best practices in treatment

 

  • Ensure active engagement in tailored care management, collaboration with members, treating providers, and their support system, to address issues, concerns, barriers to treatment, and services/supports

 

  • Ensure staff are utilizing person centered planning methods/strategies to gather information, to get to know the members supported and to adequately address specific needs of the individual

 

  • Provide support to the RCT Supervisors of Care Management and resolve issues that are escalated by Supervisors
     
  • Participate in weekly Regional Mangers meetings, Regional Leadership Team meetings, and other Department and Organization meetings Participates in Clinical Staffing and supports staff if called to attend legal proceedings

 

  • Review referrals to the Regional Care Team and checks for TCM eligibility and authorization and triages to best fit regional team

 

  • Provide support to Supervisors and staff to resolve member issues/needs in a timely manner
     
  • Facilitate team meetings, reviews team performance and areas for improvement

 

  • Works to help staff resolve billing errors and document accurately and timely
     
  • Monitor the team’s performance through the use of dashboards and reports and develops plans to improve performance

Qualifications: 

Education/Experience:
 Master’s degree in a field related to Health, Psychology, Sociology, Social Work, Nursing or another relevant Human Services area or licensure as an RN and three (3) years of experience providing care management, case management, or care coordination to complex members with I/DD or TBI.  
OR
 
Equivalent combination of education/experience.

*Prefer a minimum if two (2) years in a supervisory role of two (2) or more employees.
 *Experience working with children and adults with BH/MH/SU/IDD conditions is a plus

License/Certification:
Fully licensed by the North Carolina governing board regulating Human Services professions including: Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Addiction Specialist (LCAS), Licensed Clinical Mental Health Counselor (LCMHC), Licensed Psychological Associate (LPA), or a Registered Nurse (RN) license issued by the North Carolina Board of Nursing.

Must have a valid driver’s license
