Qualifications:

Licensed Vocational Nurse (LVN) with a valid Texas Nursing License, Experience in geriatric home health care, Strong organizational and self-management skills, Excellent customer service and communication skills.

Key responsabilities:

Conduct skilled visits to provide quality patient care as per the RN Case Manager's care plan

Assist in the development, review, and revision of the plan of care

Observe, record, and report patient reactions to treatment and changes in condition

Provide ongoing instruction and supervision to clients and their families regarding the treatment plan.

Seeking Home Health Licensed Vocational Nurse Full-Time in the Austin, TX and surrounding areas.

Shift/Schedule: Mon-Fri 8-5 p.m. with 1/On call weekend per month for full time and we also have PRN available!

Responsibilities

Skilled Visits

Assists in development, review and revision of the plan of care.
Provides quality patient care set forth by the RN Case Manager's Care plan
Observes, records and reports reaction to treatment and any changes in client’s condition to appropriate personnel and/or physician.
Provides ongoing instruction and supervision of client, family or other health team personnel in the plan of treatment.

Requirements:

Geriatric Home Health Experience
Texas Nursing License
Strong organizational and self-management skills
Strong and compassionate customer service skills
Valid driver’s license
