Registered Nurse (RN) with a valid Texas Nursing License, Experience in Home Health, particularly with geriatrics, Strong organizational and self-management skills, Excellent customer service skills..

Key responsabilities:

Provide quality nursing care in patients' homes as per the physician's plan of care.

Collaborate with the interdisciplinary team to develop and manage the plan of care.

Evaluate patient services and progress, reviewing findings with the care team and family members.

Perform PRN visits and be available on call as required.

Responsibilities

To provide quality, comprehensive nursing care in the Patient's home as prescribed by the physician in the plan of care.
Responsible for collaborating with the interdisciplinary team to develop the plan of care for the Patient.
Manage nursing treatments and activities to meet the needs of each individual patient.
Responsible for evaluating utilization of patient services and patient progress. Review evaluation with the care team, patient, family members, physician, and case manager as needed.
Performs PRN visits, and available on call as required (about every 3-4 weeks).
Other duties as assigned.

Requirements

Home Health (Geriatrics) experience
Texas Nursing License
Strong organizational and self-management skills
Strong and compassionate customer service skills
Valid driver’s license
