As a Care Navigator, your main goal will be to give our clients the best experience possible by being the welcoming face of the company. You will schedule appointments, help clients in the onboarding process, answer client questions and concerns, and do other tasks that support our clinicians as they support our clients.
Your role at ReKlame Health would include:
Contracting 40 hours a week on tasks assigned to you according to the job description
Manage office telephone system, including answering incoming calls and returning messages
Scheduling appointments for our clients and practitioners
Close monitorization for last minute rescheduling of appointments for therapy; ensure clients are booked for their next appointment.
Managing clinician calendars
Entering client personal health information and other clinical data
Conduct high-risk screenings
Run prescription history reports through prescription monitoring platform
Eligibility checks for prescription refills
Locating pharmacies in the event medication is out of stock at the patient’s usual pharmacy
Submitting lab requests (EKG, psychological, pharmacogenetic, etc)
Responding to prior authorization, form, letter and other paperwork requests
Collaborating care with external providers
Monitor and help facilitate client onboarding, ensuring client intakes are submitted and fully completed prior to the first visit
