Job Title: Physician/Nurse Practitioner for Health Risk Assessments
Job Overview
Medigence Health is actively seeking dedicated Physicians and Nurse Practitioners to conduct comprehensive Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace beneficiaries. This role offers an exciting opportunity to work remotely and travel extensively across various counties in Wisconsin, providing essential assessments in home settings, clinics, or via telemedicine.
Key Responsibilities
Conduct thorough evaluations of medical and social health for beneficiary members utilizing the Medigence Health tablet-based assessment tool.
Identify and refer patients for routine, urgent, and emergent case management.
Provide follow-up recommendations for patients to connect with their primary care physicians.
Travel to designated counties where you hold a current license, ensuring accessibility and high-quality care for diverse communities.
Engage with patients in a variety of environments, including home visits, clinics, and long-term care facilities.
Required Skills
Minimum of 2 years of hands-on experience as a Nurse Practitioner or Physician.
Knowledge of the Hierarchical Condition Categories (HCC) model, Healthcare Effectiveness Data and Information Set (HEDIS), and STAR quality measures.
Current, unrestricted state license or capability to obtain one.
Registration with Medicare and Wisconsin Medicaid.
Board certification in internal medicine, geriatric, or family medicine specialty.
Strong communication skills to interact effectively with elderly and chronically ill patients.
Proficient in the use of computers, tablets, and mobile technology, alongside comfort in using telehealth solutions.
Qualifications
Completed graduate degree in Nursing or Medicine.
Board certification in a relevant specialty preferred.
