The SDOH Navigator is responsible for implementing a community-based social determinants of health navigation program, designed to connect Community Plan of Idaho members with appropriate resources and support services to successfully maintain living in their community.
This is an Idaho-based position and you will have the flexibility to work remotely* as you take on this challenging and rewarding work.
Primary Responsibilities
Collaborate within the Community Plan of Idaho and national UnitedHealthcare teams to develop a community-based SDoH coordination strategy
Assist in the management of a program to better connect individuals with appropriate SDoH support services including developing a plan of intervention that addresses needs such as affordable housing, non-emergency medical transportation, food insecurity, and other identified SDoH needs
Initiate and develop relationships with SDoH-focused community organizations, and other service providers to identify new and existing opportunities for accessing resources
Conduct screening interviews, complete intake documentation and coordinate the location of appropriate SDoH support services
Implement processes to manage the intake and referral of individuals experiencing a SDoH need
Engage with individual member cases to identify appropriate housing (such as subsidized housing, Section 8, affordable and market rate housing) or housing support services (such as rental assistance, financial counseling, foreclosure mitigation, etc.) to place or maintain affordable / appropriate housing. This includes
the identification of need and dissemination of referral materials
Collaborate with state agencies, community organizations and other social service agencies to promote and grow SDoH opportunities within the region and / or state
Coordinate with other external social determinant of health coordinators (e.g. employment, transportation, financial services, etc.) to facilitate access to support services to sponsor successful community residency and whole person coordination

Required Qualifications
2+ years of experience in writing business and / or system requirements and business/finance/grants analysis
2+ years of experience with documenting processes and practices, often in accepted project methodology artifacts, while effectively prioritizing multiple tasks, priorities, projects, and deadline
Solid understanding of low income and / or Medicaid and/or Medicare populations with complex needs and supports, including individuals experiencing chronic homelessness and behavioral health conditions
Sensitivity to individual needs and solid person-centered orientation
Demonstrated ability to effectively network, build connections and develop solid relationships in the community
Demonstrated solid leadership skills and the ability to work independently and with minimal direction
Demonstrated solid communication and presentation skills are required for this position, as well as process development and organization skills
Demonstrated resiliency and tenacity to drive to solutions including the ability to manage conflict, influencing without authority, and the ability to problem solve beyond prescriptive solutions
Demonstrated ability to work in a complex matrix environment, influencing others through communication and organizational skills
Ability to travel locally up to 25% of the time

Preferred Qualifications

Understanding of Medicaid and Medicare programs, as well as managed care

The salary range for this role is $71,600 to $140,600 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
