The American Heart Association has an excellent opportunity for a Program Implementation Manager, Public Health based out of our National Center office! This position can be home based.

 

This is a full time, benefits eligible, grant funded opportunity. Current funding will expire on June 30, 2028, with the possibility of extension.

 

This individual will be a market and system account lead for the national Cardiovascular Kidney Metabolic Health (CKMH) initiative. Lead and coordinate initiative activities and commitments across a cohort of specific markets. Recruit health systems and stakeholders within each market, establish collaborative workgroups, contract with sites for initiative, and support inpatient and outpatient registry participation. Responsible for working with multiple health systems on improvement in treatment to guidelines, clinical processes, and practices. Responsible for communicating project status, and delivering requirements within budget, scope, and timeline. This position will also work closely with several different departments within the national office to ensure success.

Responsibilities:
  • Researches and identifies healthcare institutions/systems to target for initiative participation. Works closely with designated market team on all aspects of health system recruitment and onboarding, including strategy. Build strong relationships with health systems in assigned markets.
  • Provide consultation, direction, and support through learning cohorts to identify strategies for improving outcomes. Build relationships with internal and external stakeholders and volunteers. Facilitate learning cohort meetings in coordination with volunteers and staff partners.
  • Works closely with designated market team to develop program implementation strategies in alignment with initiative objectives. Serve as point person for ongoing tools and resources needed by hospitals and clinics to sustain knowledge (via online education, articles, etc.). In coordination with staff partners and volunteers, establish sustainability plan for all components of initiative.
  • Develop clinical decision support tools and work collaboratively with clinicians and other healthcare professionals to implement systematic efforts in improving quality of care. Work with inpatient and outpatient clinical care teams to address barriers to guideline adoption and enhance clinical workflows for improved patient outcomes.
  • Works with health system participants to assess and establish health system interdisciplinary teams, promote engagement, and foster collaboration
  • Communicate regularly with Senior Program Implementation Manager to ensure program and project-specific tasks are completed according to grant timeline. Work with cross-functional teams to build buy-in and ensure grant deliverables are met.
  • Manage project documents, including budgets, timelines, and reports on status for internal and external audiences.
Qualifications:
  • Bachelor’s Degree or equivalent experience
  • Three (3) – Five (5) years of relevant experience
  • Experience with account management and negotiation skills
  • Experience building and managing relationships with external alliances, partners, key volunteers and/or clinical stakeholders in inpatient and/or outpatient settings
  • Proficiency in project management including management of budgets, timelines, activity plans and communication with team members and stakeholders.
  • Skilled working with clinical audiences to impact change management related to patient outcomes and/or quality improvement methods/tools and processes to drive system implementation
  • Ability to interact and engage with C-Suite and/or clinical leaderships ideally in hospital or outpatient clinic settings
  • Ability to work independently and effectively manage multiple projects and detailed implementation plans in a fast-paced environment
  • Ability to work in a team environment and interact with all levels of American Heart Association staff, volunteers and the public
  • Excellent English verbal, written, and presentation communications skills
  • Ability to travel up to 20% local and overnight stay

 

Preferred Experience:

  • Experience in healthcare or in a public health-related organization: clinical, quality improvement, program development, health equity, and systems improvement.
  • Experience coaching teams in evidence-based practice models and frameworks within hospital or other clinical settings
  • Experience leading regional or national quality improvement initiatives, learning collaboratives, and/or establishing health system multidisciplinary teams
  • Healthcare quality certification and/or project management certification: Certified Professional in Healthcare Quality (CPHQ), Certification in Health Care Quality Management (CHCQM), Project Management Professional (PMP)
Compensation & Benefits:

The expected pay range will be $75,000 to $89,000. Pay is commensurate with experience; geographic differentials to the pay range may apply. The American Heart Association reserves the right to pay more or less than the posted range. The American Heart Association invests in its people.
