The Vice President, Population Health will lead the strategic development and execution of population health initiatives at Interwell Health. This role is responsible for defining a centralized population health strategy, integrating analytics and clinical insights, and driving measurable improvements in patient care, quality outcomes, and cost efficiency. The VP will play a critical role in shaping and unifying the organization’s approach to population health, fostering collaboration across teams, and ensuring the successful implementation of key initiatives. The VP will partner closely with clinical, operational, and data teams to ensure a streamlined, data-driven approach to population health management.  

The ideal candidate brings deep expertise in value-based care (VBC), population health analytics, and clinical program development and is an influential leader capable of fostering alignment, accountability, and cross-functional collaboration.  

What You’ll Do:

Strategic Leadership & Program Development

  • Define and execute a centralized population health strategy, ensuring alignment with organizational goals and VBC principles.  
  • Develop and oversee a Center of Excellence (COE) for population health, creating standardized processes for assessing, prioritizing, and implementing initiatives.  
  • Serve as the thought partner, ensuring programs are grounded in evidence-based best practices.  
  • Partner with senior leadership, clinical stakeholders, and external partners to drive system-wide population health initiatives.  

Data-Driven Decision Making & Quality Improvement  

  • Utilize data analytics, statistical modeling, and predictive analytics to assess performance trends, identify opportunities for improvement, and inform program development.  
  • Collaborate with internal clinical, operational, and data teams to integrate population health analytics into decision-making and workflow optimization.  
  • Evaluate the effectiveness of population health programs, measuring impact on patient satisfaction, healthcare utilization, cost savings, and clinical outcomes.  
  • Develop and oversee an ROI-driven approach to assessing and prioritizing new initiatives.  

Cross-Functional Collaboration & Organizational Influence  

  • Act as a trusted leader and advisor, fostering strong relationships across departments to ensure buy-in and successful execution of population health initiatives.  
  • Work closely with care management teams to prioritize interventions, allocate resources, and tailor care plans for high-risk and complex patients.  
  • Advocate for cross-functional alignment, ensuring population health efforts are streamlined and effectively executed across the organization.  
  • Assess resource needs and build a high-performing team to support population health objectives.  

What You’ll Need: 

  • 10+ years of progressive leadership experience in population health, value-based care, clinical operations, or a related field.  
  • Bachelor’s degree in public health, healthcare administration, nursing, or related field; Advanced degree (e.g., MPH, MBA, MHA, or clinical degree such as MD, DO, RN) preferred.  
  • Experience in health system, ACO, or health plan environments, with a deep understanding of quality metrics, clinical outcomes, and risk-based payment models preferred.  
  • Demonstrated expertise in value-based care models, including their impact on quality, outcomes, and cost.  
  • Proven ability to lead large-scale, cross-functional population health initiatives, including strategy development and program execution.  
  • Ability to assess, prioritize, and implement high-impact initiatives, balancing short-term needs with long-term strategic goals.  
  • Exceptional leadership, communication, and stakeholder management skills, with a track record of influencing across complex healthcare organizations.
