Cost Center: 86/Provider Based Care Management

FLSA Status: Exempt

Posting Salary Range: $66,306 - $76,915

Office Location: Remote within North Carolina.

Trillium is excitedly seeking a Population Health Manager. This role oversees the Population Health Nurse Team, providing administrative oversight and clinical supervision. The Population Health Manager supports the team in delivering critical services such as care management, care coordination, linkage/referrals, and discharge planning.

On a typical day, you might:

  • Monitor and oversee the activities of the Nurse team to ensure the needs of members, families and stakeholders are addressed appropriately, expeditiously and in a courteous manner. 
     
  • Provide coaching and education to employees to ensure continuous improvement of performance.
     
  • Work with Unit staff and staff from other departments or external partners to prepare for various reviews or monitoring.
     
  • Review, revise or develop procedures, processes, protocols, workflows and training material.

Qualifications: 

Required Education/Experience:
 
Associate’s degree in Nursing and three (3) years of experience providing care to MH/SU/IDD population and a minimum of three (3) years as an RN.

OR

Bachelor’s degree in Nursing and a minimum of one (1) year of experience providing care to MH/SU/IDD population and a minimum of one (1) year as an RN.

OR

Equivalent combination of higher education/experience.

 

Preferred Experience:
 
Experience working with acute care discharge planning and utilization management including mental health and physical health care needs, and/or coordinating physical healthcare needs is preferred. Additionally, a minimum of three (3) years of experience providing direct supervision of staff of two (2 or more employees that provide MH/SU and/or IDD services, is strongly preferred.

Required License/Certification: 

- Licensed in North Carolina by the State Board of Nursing as a Registered Nurse (RN). 
 - Must have a valid driver’s license

 Remote within North Carolina.

Must reside in North Carolina to be considered for remote status
