This position is an amazing opportunity for a board-certified nephrologist to serve as the lead clinician in the Devoted Medical Chronic Kidney Disease (CKD) Clinic. Devoted Medical is building a next generation, value based, virtual-first CKD clinic that will dramatically improve outcomes for America's most vulnerable seniors. The clinic will focus on delaying time to progression of CKD 4, 5, and ESRD, and increasing the proportion of planned rather than emergency dialysis starts for Devoted Medical patients.
The Chronic Kidney Disease Clinic is one of several of Devoted Medical’s virtual specialty care programs that are designed as “micro centers of excellence” that deliver highly tailored, specialized care to patients with specific chronic conditions.
We are looking for a forward-thinking, innovative, clinically excellent nephrologist to deliver high quality, high impact kidney care to Devoted Health plan members with CKD. You will join Devoted Medical Specialty Care as the inaugural nephrology attending.
Your primary focus will be to deliver direct nephrology care to CKD patients to ensure medication optimization, manage electrolytes and volume status, facilitate shared decision making around goals of care, and ensure effective patient centered renal replacement planning.
You will also serve as the supervising physician for the Devoted Medical Chronic Kidney Disease (CKD) clinical team. In this capacity, you will support the nurse practitioners and social work team with clinical supervision as needed.
You will serve as the key subject matter expert as we build out our CKD care model. You will also serve as the clinical content expert as we build our capacity to support CKD patients' transition to conservative care, taking full advantage of the Devoted Medical palliative care clinic.
Your clinical team will consist of an interdisciplinary team of nurse practitioners, clinical social workers, pharmacists and care navigators.
You will participate in regular panel review discussions to offer advice and provide guidance around medical management.
You will join a vibrant, dynamic Devoted Medical specialty care team of around 100 health care professionals across six clinical programs, all of whom embody a culture of treating every patient like their own family.
Required skills and experience:
You believe strongly in an innovative, value-based approach to kidney care focused on clinical quality, patient experience, shared decision making, and improved access to nephrology services.
You are board certified in Internal Medicine and Board Certified or Board Eligible in nephrology (new fellowship graduates are welcome).
You must be comfortable delivering virtual-first nephrology care through telehealth appointments, remote monitoring, in-home diagnostics, and collaboration with community nephrologists and primary care providers (PCPs).
You have an agile mindset open to a high impact, novel care model which may undergo rapid iterations to achieve continued improvements in clinical and quality outcomes.
You have a builder mentality and are excited to be the first physician in the door to build a kidney care model that lets us treat every member as if they were family.
Fluency in Spanish is a plus though not required.
Prior experience practicing in value based care models.
Willingness to obtain licensure in all Devoted Health markets. Devoted Medical offers concierge support for the licensing process.

Your Responsibilities and Impact will include:
You will provide best-in-class direct telehealth care in our virtual CKD clinic. The breakdown of your time will approximate:
Expect ample time for visits in this value-based model with initial visits lasting 45-60 minutes.
You will utilize and help improve our home-grown technology and electronic health information platform to carry out these visits.
There are no in-person clinic based or hospital based responsibilities.
Urgent calls after hours will be triaged to our 24/7 care on demand line. There are no overnight call responsibilities. You will be asked to help serve as a backup to the nurse practitioners on call for our care on demand clinic for approximately one weekend per quarter.
While our clinic is virtual, you will partner with our care coordination team to help members access high-quality nephrology and vascular when in-person care is required or preferred.
While you will be focused on the delivery of nephrology care, you will be a core part of helping iterate the clinical model to achieve better outcomes for our members in line with the Devoted mission of treating every member like family.
Salary Range: $210,000- $315,000 / year

The actual base salary offered may depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
