Job Description

This position represents an amazing opportunity for a nurse practitioner (APRN) with specialization in nephrology who has a big heart and entrepreneurial spirit and is committed to improving outcomes for people with chronic kidney disease.
Devoted Medical is building a next generation, value based, virtual-first CKD clinic that dramatically expands access to specialty care for America's most vulnerable seniors. The clinic will focus on delaying time to progression of CKD 4, 5, and ESRD, and increasing the proportion of planned rather than emergency dialysis starts for Devoted Medical patients. The Chronic Kidney Disease Clinic is one of several of Devoted Medical’s virtual specialty care programs that are designed as “micro centers of excellence” that deliver highly tailored, specialized care to patients with specific chronic conditions– e.g. diabetes, congestive heart failure, hypertension, chronic obstructive pulmonary disease, etc.
Responsibilities will include:
Your primary focus will be to deliver direct nephrology care to CKD patients to ensure medication optimization, manage electrolytes and volume status, facilitate shared decision making around goals of care, and ensure effective planning for vascular access and dialysis initiation.
You will collaborate closely with other members of the nephrology care team including physicians, social workers, pharmacists, clinical assistants. You will interface closely with community nephrologists as well as family members and caregivers to coordinate holistic care for the member, ensure continuity of care and deliver a collaborative care plan.
You will participate in regular panel review discussions to offer advice and provide guidance around medical management.
Perform comprehensive assessments including comprehensive diagnosis/disease review, medication review, and assessment for quality of care (STARS/HEDIS) interventions as well as social and home health/DME needs.
Utilize our home grown electronic health information system for visits while also providing feedback on how to improve the interface.
Maintain accurate and up-to-date patient records, ensuring compliance with relevant legal and ethical guidelines.
Participate in quality improvement initiatives and ongoing professional development to stay current on best practices and advancements in kidney care.
Adhere to all relevant laws, regulations, and industry standards, including patient privacy and telehealth regulations.
Conduct urgent visits during on-call shifts to support our clinical nurses who triage calls from our members. At least one four hour on-call shift on a weekend day or holiday is required per quarter.
Attributes to success:
Skilled nurse practitioner with thorough understanding of nephrology management including disease process, treatment modalities, medication management and lifestyle modification. Direct experience working in a nephrology practice and/or a hemodialysis center is a must.
You are experienced working on an interprofessional team and enjoy team-based care.

Desired skills and experience:
APRN with 5 or more years working in outpatient clinical practice.
Minimum of 2 years of experience in a nephrology practice or hemodialysis unit required.
Proficiency in using telehealth technology and electronic health records (EHR).
Virtual care experience is preferred along with a strong desire to continue practicing clinical nursing and performing virtual visits - you believe in the mission of bringing care to where the patient lives.
An understanding of managed care is a plus, including how to appropriately assess STARS/HEDIS measures, code clinical comorbidities, and identify clinical care gaps.
Proficiency in Spanish is desired, though not required for this position.
Multi-state licensure is required in addition to a willingness to obtain, and maintain, additional licensure as requested.
Licensure and Certification:
Master's or Doctoral degree in Nursing with a specialization in nephrology care.
An active and clear RN and APRN license as well as APRN certification is required at time of hire and must be maintained while employed at Devoted Medical.
Active BLS is required at time of hire and must be maintained while employed at Devoted Medical.
Salary Range: $120,000-$130,000 annually

The actual base salary offered may depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
