Included Health is looking for a Family Nurse Practitioners who is passionate about caring for members holistically through their healthcare journey and ensuring needs are met with industry-leading interventions, within our Care & Case Management (CCM) Program.
In this role within our Care & Case Management (CCM) Program, you'll be a key player in supporting members on their healthcare journey as part of a multidisciplinary clinical team. This team includes a Nurse Care Manager, Dietician, Pharmacist, Social Worker, and other healthcare professionals, all working together to deliver integrated remote care and case management. You'll play a vital role in collaboration by providing expert clinical guidance and working alongside the treating team to ensure the best possible outcomes for our members.
ResponsibilitiesDevelop personalized care plans by collaborating with the care team to address each member's unique health journey, including but not limited to newborn and pediatric behavioral health.Empower patients by providing expert clinical guidance and education on health-related concerns to foster informed decision-making.Become a trusted resource by conducting compassionate phone consultations with active listening, clear answers, and supportive guidance.Advocate for patient well-being by ensuring members navigate available resources effectively for improved health outcomes.Partner with local treating teams and employer-provided benefits to help patients seamlessly utilize their care and clinical resources.Drive program excellence by analyzing member data and clinical trends to identify areas for program improvement.Champion virtual navigation by facilitating coordinated and compassionate clinical navigation via phone or video.Craft impactful care plans by partnering with members, care managers, and the supporting multidisciplinary care team.Ensure seamless care transitions by collaborating with local providers to maintain continuity of care throughout the patient's journey.Engage in holistic problem-solving by coordinating necessary resources (clinical and social) to comprehensively address members' concerns.Elevate care quality by providing in-depth clinical expertise to enhance each member's health experience.Shape the future of care by offering thought leadership in the care management program.Continue learning by partnering with the Credentialing Team to expand practice scope through additional state licensing and supervisory agreements (if applicable).Perform other duties as assigned.Active Family Nurse Practitioner license with Board Certification requiredReside in RN compact stateAt least 3-5 years of post-graduate experience in family medicine, with an assigned panel of patients, treating both adult and pediatric patientsWillingness to see patients across the lifespanWillingness to become and maintain licensure in multiple states to meet program needsPreferred QualificationsHolds multiple state licensesProficient in delivering comprehensive medical care across all life stages, with strong emphasis on newborns and complex pediatric medical and behavioral needsExperienced in providing telehealth services, utilizing digital platforms to deliver remote medical care and consultationsPhysical/Cognitive Requirements:Be highly empathetic. We work with members and their families who are going through challenging times. Ideal candidates practice empathy and reassure members that we are available to help them.Ability to understand cultural and socioeconomic issues affecting members and to coordinate all available resources to serve members. Collaborate well across multidisciplinary teams with clinical and non-clinical members to deliver a seamless, top-quality experience to members.Strong verbal and written communication skills. You will spend time on the phone with members and families, as well as a lot of time communicating with colleagues. Therefore, the ability to gather a clinical history, answer questions at a member level, and succinctly summarize findings is critical.Must be able to work efficiently. We are a fast growing company and we are busy. Our team is expected to meet volume goals without sacrificing quality. Good judgment for balancing priorities is a must. Be flexible and comfortable with working in a rapidly-changing environment. Strictly follow security and HIPAA regulations to protect our members’ medical information.
Strong competence and ability to use multiple computer/medical record systems. Prompt and regular attendance at assigned work location.Ability to remain seated in a stationary position for prolonged periods.Requires eye-hand coordination and manual dexterity sufficient to operate keyboard, computer and other office-related equipment.No heavy lifting is expected, though occasional exertion of about 20 lbs. of force (e.g., lifting a computer / laptop) may be required.Ability to interact with leadership, employees, and members in an appropriate manner.

The United States new hire base salary target ranges for this full-time position are:
Zone A: $108,890 - $141,560 + equity + benefits
Zone B: $125,224 - $162,794+ equity + benefits
Zone C: $136,113 - $176,950 + equity + benefits
Zone D: $141,557 - $184,028 + equity + benefits
This range reflects the minimum and maximum target for new hire salaries for candidates based on their respective Zone. Below is additional information on Included Health's commitment to maintaining transparent and equitable compensation practices across our distinct geographic zones.
Starting base salary for the successful candidate will depend on several job-related factors, unique to each candidate, which may include, but not limited to, education; training; skill set; years and depth of experience; certifications and licensure; business needs; internal peer equity; organizational considerations; and alignment with geographic and market data. Compensation structures and ranges are tailored to each zone's unique market conditions to ensure that all employees receive fair and competitive compensation based on their roles and locations.
