Job description

Arkos Health is seeking a Home Wellness Provider to join our team! This position is a virtual telehealth role and is offered per diem. Open to MD /DO.

Please note: Must reside and be fully authorized to work in the US.  **We are seeking remote applicants, preferably in Arizona, Georgia, Illinois, Montana, New Mexico, Nevada, Texas, Vermont or any state except California, Colorado, New Jersey, New York, Oregon, Massachusetts and Washington.

The ideal candidate for this role demonstrates a high level of clinical expertise in working with members with acute and chronic conditions with the ability to work autonomously.  The Home Wellness Provider will be responsible for assessment, diagnosis, treatment, plan, management, education, health promotion and care coordination for member health needs on a virtual basis. They will provide support for the achievement of optimal well-being of Arkos Health patients in consideration of their family, community, and environmental backgrounds. The Home Wellness Provider  will use their skillset, and experience to provide care within the scope of practice as outlined by applicable state law, licensing, and regulations.

Essential Duties and Responsibilities:

  • Performs comprehensive health assessments that are complete, accurate, relevant and appropriate for age, gender and clinical presentation and/or problem based on referral visit type for members served.
  • Reviewing medical records prior to visit
    • Demonstrates knowledge of etiologies, risk factors, pathophysiology, and presentation of acute and chronic conditions
    • Conducts thorough review of available member records in preparation for scheduled visit to assist in supporting capture of appropriate diagnoses, while utilizing critical thinking to interpret diagnostic studies and screening evaluations to develop appropriate diagnosis and plan based on results
    • Develops, implements, evaluates, and alters treatment plan of care as needed
  • Discussing appropriate history and health related information such as social determinants of health from patient
    • Makes recommendations and/or adjustments to medications as needed
  • Demonstrates and maintains accurate, complete, and timely documentation in electronic health record to include but not limited to:
    • Correct diagnosis code(s)
    • Status of all acute and chronic conditions identified
    • Plan to address all diagnosis identified at time of visit
    • Demonstrates understanding and working knowledge of key elements of Risk Adjustment Factor (RAF) and Hierarchical Condition Categories (HCC) model within 30 days of hire
    • Demonstrates understanding and working knowledge of key elements of Quality STAR/HEDIS measures within 30 days of hire


    Qualifications and Skills:

    • MD or DO degree 
    • Current IL Medical License and will provide care within the scope of practice as outlined by applicable state law, licensing, and regulations
    • Experience working with adult, older adult, geriatric population preferred
    • Experience and understanding of population health management preferred
    • Computer and technical skills to perform virtual assessments, utilize member system and Microsoft applications
    • Able to be credentialed with local insurance companies
