KEY JOB RESPONSIBILITIES: The Clinical Educator is a professional, Registered Nurse responsible for all the training/ educating of the clinical team for home health and hospice. Clinical Educator has accountabilities for the monitoring, implementation, and evaluation of clinical training and competencies.

Additionally, the Clinical Educator will:

  • Participates as a member of the education steering committee to develops, implement, evaluate, and oversee The Care Teams Education Program.
  • Collaborates with Education Steering committee, Branch Director, HR and Clinical Team Leads to provide a process to promote and ensure employee orientation, competency, and in-service education programs.
  • Provides direction and coordination of education in-service.
  • Assists in the identification of skills for annual competence and provides in-services.
  • Maintains aggregate records of employee orientation and annual competency requirements as part of the plan.
  • Responsible for the implementation and monitoring of competencies.
  • Educates staff from onboarding, orientation, and initial training.
  • Ensures initial competency as well as annual is completed accurately and timely within organization policy.
  • Outreach to welcome new hires prior to start date to introduce and talk them through orientation process and timeline.
  • Assignment of appropriate knowledge tracks in any applicable ancillary training systems.
  • Continuous (weekly) check in with new hire to ensure the completion of all LMS, EMR specific and other orientation education requirements.
  • Facilitate the completion of the Clinical Skills assessments (as applicable), including the update to acknowledge the completion in the EMR and HR Systems.
  • Responsible for the timely escalation and ongoing plans as needed to Branch Directors when new hires are falling short of the required expectations for the completion of education and clinical orientation requirements.
  • Utilizes and researches a variety of training techniques in delivering both face to face and virtual presentations.
  • Collaborates with the Compliance Managers regarding policy and process development and/or revision as it relates to maintaining regulatory compliance or educational needs.
  • Maintains current knowledge of Federal and State healthcare regulations and program requirements.
  • Regularly participates in case conference activities and assures through attendance and record reviews that conferences are conducted in accordance with Agency and CMS regulations.
  • Establishes and maintains collaborative working relationships with all locations, providing on-going support as needed.
  • Participants in the agencys ongoing audits to evaluate need for employee education and/or re-education.
  • Participates in the development, implementation, and monitoring of the Agencys Quality Assurance and Performance Improvement Program (QAPI).
  • Participation in Quality Assurance/ Performance Improvement Program TCT sponsored training.
  • Works in collaboration with Branch Directors and Clinical Team Leads to assure clinical documentation is accurate and completed within a timely manner.
  • Educates and implements monitoring activities for safety, risk management and infection control.
  • Coordinates training for new hires; to include EMR training, orientation books and clinical supplies.
  • Acts as resource to personnel in education of clinical procedures.
  • Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal and professional network and participates in professional organizations or societies.
  • EMR and documentation review training and orienting to include assisting quality assurance with chart audits, clinical authorizations review and auditing compliancy for patients.
  • Works with Branch Director and Clinical Team Leads and staff clinicians to gather, collate, and report patient care summary information to Case Managers.
  • Works with the Branch Director, Clinical Team Leads and Scheduling teams to ensure proper staff utilization and confirms that consents and NMNOC's are plotted correctly on start of care as needed.

Regulatory Requirements

  • Registered Nurse with current licensure to practice professional nursing in the state
  • Bachelor of Science in Nursing (BSN)
  • Masters degree in Nursing or healthcare-related field preferred
  • 2 years of experience in Hospice and Home Health Education preferred
  • 1 year of experience in Quality Improvement related activities preferred
  • Superuser in the EMR system
