About the Position

Responsibilities

  • Design and maintain modular care plans for diabetes education alongside the clinical leadership team. 

  • Lead a growing team of Certified Diabetes Care and Education Specialists (CDCES) or Certified Diabetes Educators (CDE)

  • Obtain histories, conduct virtual visits with patients, assess diabetes management needs, set goals, and create personalized diabetes education plans.

  • Educate patients on diabetes self-management skills, including blood glucose monitoring, insulin administration, medication adherence, nutrition, and exercise.

  • Utilize motivational interviewing and behavior-change strategies to support patients in achieving diabetes management goals.

  • Monitor patient progress, follow up on patient concerns, and make adjustments to care plans as needed.

  • Works collaboratively with other providers, including using comprehensive charting and messaging to ensure a comprehensive approach to diabetes management.

  • Communicates effectively with other providers, including using comprehensive charting and messaging.

  • Delivers patient care that balances the best that medical science has to offer, patient preferences and values, and invests in patients' future confidence and skills.

  • Provide feedback about opportunities to improve the quality of care and operational processes.

  • Stay current with diabetes care best practices, tools, and telemedicine innovations to continually improve patient support and outcomes. 



Requirements
  • REQUIRED: Certification as a Certified Diabetes Care and Education Specialist (CDCES) or Certified Diabetes Educator (CDE)

  • Acceptable Licensure: 

    • Registered Nurse (RN)

  • REQUIRED: Minimum of 2 years of experience working with patients with diabetes, with at least a portion of this in a telemedicine setting.

  • REQUIRED: Strong knowledge of diabetes management, medications, education, lifestyle interventions, and self-care strategies.

  • Previous experience in a role as an educator, working within multidisciplinary teams and/or a leadership position preferred.

  • Excellent interpersonal and communication skills to effectively support patients in a virtual setting.

  • Telemedicine or virtual care experience is a plus.

  • Competent in providing patient education at every visit.

  • Proficiency with technology and assisting patients with troubleshooting technical difficulties that may occur in the home environment.

  • Adept with Google Suite and multiple Electronic Medical Record (EMR) systems. Experience with Zoho is a plus. 

  • Timely and professional.

  • Compassionate, patient-focused attitude with a commitment to improving patients' quality of life.

  • Fluency in Spanish (both written and spoken) is a plus.



Benefits
  • Pay type: $90-100K salary 

  • This is a W2, full-time position that will report to the Medical Director.
