This is a Telehealth Nurse Practitioner role where you will manage a cohort of patients and leverage our technology to help patients with diabetes, hypertension, and other chronic conditions live a healthier life. We are looking for an incredibly patient-focused nurse practitioner to provide world class care that patients deserve but can’t get in the current healthcare system.

Active Nurse Practitioner State Licenses in 3+ of following states: Virginia, Texas, North Carolina, Florida, Georgia, Ohio

What you’ll be doing:

Provide world-class care to patients leveraging data to inform your decision-making.
Conduct live and interactive telehealth clinical assessments of patients resulting in recommended treatment plans.
Build trust with patients and ensure the care that is provided is not only in line with CopilotIQ’s mission but also compassionate.
Work closely with our telehealth nurses and collaborating physicians to coordinate care.
Support new patient enrollment as well as ongoing care for CopilotIQ patients.
Document all encounters and patient notes in AthenaHealth EHR.
As our company scales, opportunity to assist in the implementation of changes to our systems and procedures.
Special projects and other assignments as needed.

What you’ll bring:

5+ years of Nurse Practitioner (NP) experience; telehealth and adult chronic care management experience is strongly preferred.
Completed an accredited FNP program with a national certification.
Active NP / Nurse Practitioner State License required in 3 or more of the following states: Virginia, Texas, North Carolina, Florida, Georgia, Ohio
Willingness to obtain additional state licenses as needed.
Ability to become credentialed with Medicare and Medicaid, and other payers as necessary.
Board Certification required.
