Job Description

The main responsibilities of the Clinical Pharmacist include medication review/management for members in select health plans, management of pharmacy-related Star/HEDIS measures, providing pharmacy support to internal and external clients, providing coverage for clinical and non-clinical functions performed by a pharmacist, and assisting in pharmacy projects.

Responsibilities

  • Provide coverage for clinical and non-clinical functions performed by pharmacists including but not limited to: 1) medication reconciliation for post-discharge members to avoid potential drug related issues including duplicate therapies or gaps in therapy; 2) pharmacy consultations for patients enrolled in the disease management programs for anticoagulation, CHF, COPD, diabetes, or CKD; 3) Key events & ICT medication reviews to address any drug-related issues and educate members on appropriate utilization of drugs; 4) medication dispensing in an outpatient pharmacy.
  • Medication Management - Conduct medication reviews to identify and resolve issues with appropriate documentation in a central case management system. Assist members to be compliant with medication regimens, arrange copay or financial assistance programs as appropriate, and provide education. Consult with physicians to resolve drug-related issues.
  • Star/HEDIS Programs - Work in collaboration with health plans to develop strategies in delivering a high STAR rating for pharmacy related measures. Implement and monitor workflows and initiatives to close clinical gaps through member and provider (physicians, pharmacies) facing interventions.
  • Provide pharmacy support to both internal (e.g., Medical Management, Network Management) and external (e.g., staff and contracted physicians and office staff) clients. Attend multidisciplinary clinical rounds as applied and work with other healthcare professionals to address drug-related issues.
  • Assist in pharmacy projects, including but not limited to 1) Provide support for the Pharmacy & Therapeutics Committee, Utilization Management Committee, and Quality Management Committee. 2) Develop utilization management protocols for drugs including injectables, infusion therapies, chemotherapeutic agents, and vaccines and internal treatment algorithms. 3) Present new drug reviews, therapeutic class reviews, drug usage guidelines, and clinical programs to the Pharmacy & Therapeutics Committee. 4) Other pharmacy related projects

Qualifications

Minimum Education: Minimum Education: High School Diploma or Equivalent.

Minimum Experience: Current knowledge and expertise in clinical pharmacology and disease management. Experience in managed care and/or clinical experience preferred.

Req. Certification/Licensure: Must hold an active pharmacist license in California.
