Job Description

The HCC Coordinator makes calls to and answers calls from providers and their staff regarding HCC Risk Adjustment Department reports and projects. The coordinator also serves as a clerical resource for other team members and management

Responsibilities

  • Assist with projects for the HCC Risk Adjustment team and other assignments received by management.
  • Research and provide courteous, accurate and timely response to inquires by providers as related to HCC Risk Adjustment projects and reports.
  • Document all phone calls regarding chart audits in IDX/CSR system on a daily basis.
  • Responsible for departmental provider report mailings to all providers.
  • Retrieving progress notes and faxes from provider relations and provider office. Distribute all incoming faxes, progress notes and chart audits.
  • Timely reporting of internal and external audit summaries to the Manager of HCC Teams.
  • Notify management of call type trends and complaints immediately with accurate details by provider.
  • Mails post cards appointment reminder and gift cards to members. Other duties as assigned by management based on departmental needs for HCC
  • Fax wellness results to back to pcp’s within a timely manner.

Qualifications

Minimum Education: A High School Diploma or Equivalent; BS/BA preferred.

Minimum Experience: At least one (1)year relevant experience within the healthcare industry. At least one year experience in creating and generating reports.
