This position remotely supports our Ohio teams



The ConcertoCare Behavioral Health Manager supports the daily activities of the behavioral health team, ensuring that processes and programs are implemented. This position serves as a resource and subject matter expert for Social Workers and Community Health Workers in their assigned geography, focusing on behavioral health staffing, training, coaching, and oversight of activities related to Social Determinants of Health (SDOH).


The role involves working closely with market-level care teams to ensure successful achievement of behavioral health/SDOH outcome measures. Additionally, the Behavioral Health Manager will perform the activities of a Social Worker as needed within an assigned geographic area, making it an ideal position for a leader who seeks to contribute to the health and wellbeing of complex patients and is enthusiastic about developing an innovative, value-based clinical model.


AS A LEADER

  • Supervise day-to-day performance of the behavioral health team in the assigned region and provide regular feedback to Social Workers and Community Health Workers
  • Recruit, train, and coach behavioral health staff in the assigned region and ensure staff are trained in evidenced-based practices for behavioral health care
  • Oversee job performance and conduct mid-year and annual performance reviews for the team.
  • Generate reports and monitor performance against goals for markets in the region, ensuring high quality and timely implementation of activities.
  • Promote staff growth and development by identifying educational opportunities, conducting training sessions, and modifying policies and procedures to address changing strategic objectives.


AS A PARTNER

  • Collaborate with the VP of Behavioral Health to develop and implement strategies for achieving behavioral health goals.
  • Design behavioral health programs and documentation requirements in partnership with the VP.
  • Partners with the Project Management Staff, IT office and other corporate teams as the Subject Matter Expert for behavioral health initiatives and in support of special projects.
  • Facilitate communication between clinical team members and local market teams regarding treatment recommendation, rising/high risk patients and other behavioral health care management aspects.


AS A SOCIAL WORKER

  • Provide patient care for a partial case load, supporting mental and physical health based on assigned panel.
  • Screen and assess patients for various mental health conditions and participate in differential diagnosis support.
  • Provide brief behavioral interventions using evidence-based techniques such as Behavioral Activation, Problem-Solving Treatment, Motivational Interviewing, and other treatments as appropriate.
  • Document assessments, patient progress, and treatment recommendations in electronic health records. Closing all visit notes within 72 hours.
  • Facilitate treatment plan changes for patients who are not improving as expected, consult with the primary care provider, and coordinate referrals for more specialized mental health care when necessary.
  • Ensure care is delivered in a manner that honors patients’ diverse values, beliefs, behaviors, backgrounds, social, cultural, and linguistic needs


Required QUALIFICATIONS

  • Master’s in Social Work and an independent Social Work License in good standing in the state of practice.
  • At least 2 years’ experience with team leadership or individual mentorship.
  • Ability to train and support others as a Behavioral Health Subject Matter Expert.
  • At least 2 years working with data and reporting.
  • At least 3 years’ experience working in a clinical setting as a Social Worker.
  • Demonstrated ability to triage patient reported symptoms and issues that require escalation to our field-based team and apply critical thinking skills.
  • Ability to perform multiple tasks concurrently with minimal supervision, meet deadlines, and thrive in a fast-paced, dynamic team environment.
  • Proficient in computer skills including Microsoft Office Suite (Outlook, Excel, PowerPoint, Word) and EMR experience, as well as internet-based tools and applications.
  • Demonstrates high level of professionalism and possesses strong problem-solving, organizational, and communication skills.
  • Strong clinical skills and ability to implement evidenced-based care.
  • Ability to manage patient complexity and multiple clients with diverse needs.


Special Consideration for those that also have

  • Strong preference for experience in a medical environment.
  • Preference for those with productivity and outcomes tracking experience.
  • Preferably clinical experience could include: home health care, ambulatory care, community public health, and/or any psychiatric care facility, including both inpatient and outpatient settings.
  • Geriatric care experience highly desired.
  • Knowledge of Medicare and Medicaid populations is a plus.


Base Salary/ Wage Range $85,000 to $105,000 plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
