Are you a compassionate MSW with a passion for providing exceptional end-of-life care? In this role, you will play a crucial part in supporting patients and their families through the challenging stages of Chronic Kidney Disease (CKD) and End Stage Kidney Disease (ESKD), ensuring they receive the highest quality of care and emotional support.
- Remote Opportunity: Provide virtual patient support and lead a team of care coordinators from your home office, with occasional travel to see patients.

Key Responsibilities:
Team Leadership: Directly supervise a regional team of Care Coordinators, providing oversight of day to day operations and guidance navigating insurance benefits, community resources and support services (30% of the time)
Lead Social Worker: Work directly with patients and families (70% of the time)

Salary/ Wage Range
$56,500.00 - $83,000.00 / yearCompensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience and may fall outside of the range shown.
