Care Manager

We are looking for a Care Manager to join the Chronic Care Management team within our Product Management division. In this role, the Care Manager will work in a remote capacity to serve as an extension of the provider practice by supporting patients in between provider visits.  This individual will support patients in adhering to their personalized care plan through patient education, care coordination, motivational interviewing, and health coaching. Job Responsibilities

  • Provide care management services to a panel of patients
  • Educate patients on their chronic conditions, self-management skills and goal setting. Chronic conditions include Diabetes, CHF, COPD/Asthma, Hypertension, CAD, Ischemic Heart Disease, Anxiety, Depression.
  • Implement and update the Plan of Care by reviewing their goals, medications, upcoming appointments, care gaps, biometrics, and vital signs
  • Utilize Motivational Interviewing or other behavior change techniques to coach and assist the patient with self-management.
  • Conduct Transitional Care Management activities to high-risk patients discharged from the hospital and the ER to reduce unnecessary readmissions, Activities include medication reconciliation, medication adherence, identifying and addressing barriers, encouraging follow-up care, and how and when to seek appropriate level of care.
  • Document amount of time spent while updating patient progress and clinical notes in the athenaOne electronic health record

Typical Qualifications

  • Licensed Practical Nurse required. Compact License required as well.
  • 1+ years of experience required
  • Experience working in a care team environment
  • Prior healthcare experience serving chronically ill patients
  • Telephonic case management or care coordination experience
  • Medicare or working with the Medicare population preferred
  • Excellent written communication skills
  • Motivational Interviewing or other behavior change communication techniques preferred
  • Knowledge of medical terminology
