Job Description

This position is remote but requires in home visits across region 9. Travel may vary and the region covers California, Nevada, Arizona, and Hawaii. Applicants can be from any of these states since the position is remote.

Position Title: PRS Intensive Case Manager Level 3

Reporting Relationship: PRS Clinical Supervisor

Position Summary

Working under a grant from the Office of Refugee Resettlement (ORR), NYAP’s Post Release Services (PRS) Intensive Case Manager Level 3 provides post release services focused on stabilizing the family unit utilizing trauma-informed and intensive case management and connecting with community-based services to address concerns that require long-term care. Required travel includes initial in-home assessment followed by weekly in-person contacts for the first 45 to 60 days, then monthly in-person or virtual contacts for 6 months, depending on the needs of the family.

 

RESPONSIBILITIES

  • Perform all work in a manner consistent with the National Youth Advocate Program’s Mission, Values and Philosophies.
  • Provide Level Three (3) post release services (PRS) to children and sponsor families. As needed, PRS Intensive Case Managers may also provide Level Two (2) and Level (1) post release services.
  • Ensure delivery of intensive in-home services and engagement with children and families that provides trauma-informed and intensive case management aimed at stabilizing the family unit by utilizing intervention(s) tailored to the needs of the family, and connecting the released child and sponsor family with community-based services to address concerns that require long-term care (such as a mental health diagnosis).
  • Conduct initial in-home assessment followed by weekly in-person contacts for the first 45 to 60 days, depending on the needs of the family.
  • Maintain monthly in-person or virtual contacts for 6 months following referral acceptance.
  • Carry a minimum caseload of 3 cases per worker.
  • Ensure excellence in PRS documentation and Program compliance.
  • Actively assist in establishing working relationships with community-based organizations responsible for social services in their proposed area of operations.
  • Provide and foster a coordinated case management approach that adopts a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet the unaccompanied child and the sponsor's presenting needs.
  • Ensure proper reporting and elevation to ORR of timely notifications of concern and reporting to other stakeholders per state and ORR guidelines.
  • Provide and ensure effective communication with community partners, community-based organizations, and ORR stakeholders.
  • Demonstrate working knowledge and understanding of ORR Policy and Procedures, Flores Agreement as well as other applicable, and pertinent regulations and laws including the Trafficking Victims Protection Reauthorization Act (TVPRA) of 2008 et.al.
  • Ensure protection of Personal Identifiable Information (PII) and adhering to confidentiality of information and HIPAA regulations per ORR Policy requirements.
  • Establish a respectful, supportive workplace environment that promotes NYAP’s values of caring for people, connecting communities and promoting peace while ensuring staff accountability and compliance.
  • Elevates any issues or concerns to NYAP leadership and ORR.
  • Perform other duties as assigned.

MINIMUM QUALIFICATIONS

  • Applicant must be at least 21 years old. 
  • Master’s degree in social work, psychology, sociology, or other relevant behavioral science in which direct clinical experience is a program requirement or bachelor’s degree with at least 5 years of clinical experience. May be licensed, or eligible for licensure.
  • Highly proficient in Spanish or have access to qualified interpreters.
  • Prior experience with ORR HS/PRS preferred.
  • Proficient in the use of desktop and laptop computers, using the internet and related computer programs including ORR’s Official System of Record (also known as the UC Portal). 
  • Active motor vehicle license, reliable automobile, a good driving record, and 100/300 insurance coverage.
  • Pass a criminal history screen, including state and local child protection agency registries, and C/ANs check.
  • Willingness to work flexible and non-traditional hours in the service of persons served.
  • Candidates are required to have a reliable means of transportation and be available to travel up to 75% of the time. 

PHYSICAL DEMANDS

  • Use of manual dexterity, tactile, visual, and audio acuity
  • Use of repetitive motion, prolonged periods of sitting and standing, and sustained visual and mental applications and demands.
  • Occasional lifting (up to 25 pounds), bending, pulling, and carrying.
  • Quantitative/mathematical ability (addition, subtraction, multiplication, division, standard measurements).
