This is a remote position, you do not need to reside in NM, however, a New Mexico licensed is preferred.
Facilitates company’s efforts in the development of pharmacy solutions. Provides expert consultation and education on appropriate use of medications. Reviews prescribing patterns and provides consultation to providers regarding these patterns using evidence-based best practices. Participates in the distribution of clinical drug information and educates providers, patients, nurses, social workers, and any other treatment team members regarding the use of pharmaceutical treatment regimens. Works closely with internal staff in the refinement of the clinical program, providing consultation on pharmacy issues to clinical operations representatives and facilitating the successful implementation of these programs for customer organizations. Complies with all corporate and facility standard operating procedures, policies, guidelines, and work instructions.
- Pharmacy Data Review: Performs full medication reviews to evaluate medication related issues including drug interactions, inappropriate dosing, duplicate therapy, therapeutic alternatives, etc. Regularly reviews and monitors prescription data and prescribing practices to identify outliers by internal and/or external analysis of pharmacy claims data. Identifies opportunities for clinical interventions.
- Collaborates with other clinical personnel in assessing treatment guidelines. May participate in formulary development and management. Supports the national Pharmacy and Therapeutics Committee.
- Participates in multidisciplinary patient rounds to communicate any drug related issues and provide patient specific drug therapy recommendations.
- Consultations/Interventions: Per plan guidelines, contacts prescribers and/or patients identified as needing consultation to provide coaching on prescribing practices that have been identified as outliers. Contact may be telephonic, in-person, or through email.
- Intervention Tracking and Documentation: Tracks contact attempts and contacts per operations procedures. Documents content of and results of consultations as defined in operations procedures; maintains records as required.
- Quality Improvement: Contributes to program improvements by using continuous quality improvement processes, by being proactive in identifying improvement opportunities and by contributing on-going consultation and collaboration with product leaders.
- Medical/Clinical: Upholds clinical integrity of pharmaceutical solutions by staying up-to-date on medication information, contributing to the identification of pharmacy program best practices, collaborating with Medical Directors, Medical Dept., and Clinical Dept. and provides pharmacy expertise within the organization.
- Adheres to organizational policies and procedures. Ensures compliance with Federal and State laws and standards of practice which govern the profession of pharmacy. Maintains pharmacy records in accordance with all applicable state and federal laws and regulations. Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature, continuing education and any company provided in-services. Completes all mandatory company provided on-line courses. Attends and completes all employee development courses.
