This Nurse Coordinator (NC) role is part of a dynamic Care team within the Office of Population Health (OPH) Clinical Programs Relay Center. This position will report to the Manager, Transitions of Care. The Nurses Coordinator is a fully remote position with some requirement for travel to San Francisco or the Brisbane Office to meet Divisional priorities.
The incumbent should be able to provide a safe and private home office environment for conducting patient outreach and UCSF business.
The Nurse Coordinator will assists with navigating all aspects of the healthcare system. This position will require reporting of Subcritical Laboratory or Radiology test results to internal or external providers who ordered tests that were performed at a UCSF Health Laboratory or in the Radiology Departments. This could include follow-up test results from the Emergency Department and other ambulatory care settings. This position will collaborates with various internal and external partners to ensure that patient results are reported in the appropriate time as outlined in the UCSF Subcritical Values guidelines. Results that cannot be communicated to a provider will be communicated with the patient. The team will ensure that follow-up care is coordinated based on the guidance of UCSF resulting providers. This position requires Care Coordination for patient to find internal or external PCPs, place referrals to specialty care clinics, schedule follow-up testing and connection to community resources. The relay center team will work to meet patient expectations, achieve quality outcomes and build a patient focused culture.
Responsibilities
The Nurse Coordinator(s) will be responsible for handling all clinical escalations from the teams Health Care Navigator(s). In addition, all relay center staff will work with leadership to ensure workflow development is aligned with UCSF Health Policy and Procedures. This includes:
- Assisting with quality improvement projects aimed at tracking outcomes for the relay center.
- Ensure patient results have been communicated to the ordering internal or external provider.
- Ensure that patient who do not have primary care providers are given resources on where to obtain follow-up care. This may include working with various external clinics and community organizations to ensure access to care.
- Scheduling of follow-up appointments at UCSF around the recommended timeframe by the resulting provider.
- Refer patients to other OPH Clinical Program teams if part of the Accountable Care Organization for additional care coordination.

UCSF's Temporary Employment Program (TEP) recruits and hires temporary employees for immediate clerical and technical support services to UCSF Departments and various off-campus locations. UCSF departments deploy temporary employees to work on special projects, fill in for regular employees who are on vacation or leave, or to temporarily fill a vacant position during recruitment. Frequently temporary employees become successful candidates for career and limited appointment positions.
Required Qualifications
- Strong knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations. Knowledge of Medical Terminology. Strong knowledge of data collection, compilation, and analytical techniques.
- Strong skills to comprehend and assess patient's grievances to quickly locate appropriate resource for assistance. In-depth knowledge of the organization and how to get issues resolved.
- Demonstrates communication and problem-solving skills necessary in patient/family and healthcare team interactions
- Demonstrates knowledge of the principals of growth and development over the life-span and incorporates this knowledge into plans of care and/or goals of care.
- Excellent written and verbal communication skills.
- Has the skills to work independently in a fast-paced, demanding environment with minimal supervision.
- Ability to manage and oversee multiple tasks simultaneously, including high daily call volume Knowledge of medical terminology
- Minimum of two years of recent experience as a registered nurse caring for ill adults and older adults.
- Prior experience developing workflow procedures and implementing systems to improve operations Proficiency with EPIC APeX applications
- Bilingual/Multilingual capability
- Interest and/or experience working with pediatric and adult patients who may have cognitive difficulties
- Experience with telephone triage
Preferred Qualifications
- Proficiency with Windows-based software including Microsoft word, Excel, Outlook. Knowledge of computer systems and software used in functional area.
- Prior experience developing workflow procedures and implementing systems to improve operations
- Proficiency with EPIC APeX applications
- Bilingual/Multilingual capability
- Interest and/or experience working with pediatric and adult patients who may have cognitive difficulties
- Experience with telephone triage
License/Certification
- Graduate form an accredited school of nursing, BSN is required.
- Current CA license to practice as a Registered Nurse
- AHA Basic Life Support (BLS) certification
