Position Overview

As an RN Case Manager, you will play a crucial role in delivering high-quality care to patients in their homes. This position offers competitive pay at $45.00 per hour.

Qualifications

Minimum of one year RN experience in a Skilled Nursing environment

Current Registered Nurse (RN) license in good standing

Graduated from an accredited School of Nursing

Proficient in providing skilled nursing services in home settings, including wound vac management, IV infusion administration, blood draws, wound care assessment and dressing changes, enteral nutrition setup, medication administration, tracheostomy care, incontinence care, and comprehensive patient assessment

Ability to supervise LPNs and home health aides in compliance with regulatory guidelines

Possession of a current & valid driver's license with reliable and insured transportation

Current CPR Certification

Negative TB test/negative chest x-ray

Functional electronic device with cellular service for field charting

Familiarity with home health care electronic charting and industry regulations

Essential Functions

Provide compassionate care to multiple patients in their homes

Conduct thorough nursing assessments, including CMS OASIS documentation

Communicate significant findings to physicians and Clinical Managers

Execute physician orders accurately and promptly

Ensure safety during treatment procedures and maintain a safe environment

Supervise and evaluate care provided by nurses and aides

Educate patients and family members on new diagnoses, care techniques, and support strategies

Maintain accurate and timely documentation

Attend required in-services and meetings

Adhere to HIPAA guidelines and maintain patient confidentiality

Position requires 100% travel to patient homes
