Northwest Permanente, P.C. is currently seeking a Board Certified or Board Eligible Physician to join our growing medical group. This role will primarily focus on patients on the outer, east Portland region, out to the Gresham/Sandy/Molalla areas as part of the Home-based Palliative Care team. Hospice and Palliative Medicine (HPM) Board Eligibility is required. Completion of an HPM fellowship is strongly preferred. You will need to have excellent communication and collaboration skills, flexibility, and a good working knowledge of the field of primary care (and how it intersects with palliative care).

 

Kaiser Permanente NW has its own hospice, home-based palliative care, integrated clinic-based palliative care, and an inpatient consultation service. This is an exciting time to work in the Department of Hospice and Palliative medicine at Northwest Permanente. The opportunities for palliative care clinicians are challenging and varied, and there is broad institutional support for our mission. We place a strong emphasis on a team-first, collaborative approach with an expectation that patient care will also entail up to 50% virtual care delivery.

 

As an essential member of our hospice palliative care team, you will practice end-of-life care in a supportive, collegial environment. We prefer to have consistency for scheduling and panel management while also expecting our physicians to have some flexibility to support our patient’s needs as we grow and evolve our care delivery model. Our team prioritizes flexibility to support each other, when needed, across our robust Palliative Medicine program across our full range of patient needs. Our Palliative Care team has flexibility to work from home and conduct in-person patient visits. You will also share in our Hospice night/ weekend call with a group of 8 physicians, primarily from home.

 

All applicants must have experience with digital technologies (examples include telehealth and/or digital health). You will need to demonstrate both flexibility and adaptability to incorporate these into your practice as we continue to evolve our virtual care model.  A comfort level with concurrent documentation is also needed for this role. We use a combination of face-to-face care and the latest virtual modalities to provide evidence-based, psychiatric treatments, including primary care consults, crisis interventions and medication consultation. Our physicians will provide a combination of onsite and virtual care.

  • $205,000 to $232,800 salary (1.00 FTE)
