Description

The Intake Coordinator (IC) shall provide a person-centered approach to identify service needs and link to the most effective service/region and appropriate level of care and support for referrals. The IC works in partnership with all Lutheran Services Carolinas SC Foster Care teams to address identified needs within the catchment area. The IC will proactively respond to all SC DSS referrals. The IC, in conjunction with the regional program, will coordinate in a timely manner intake and admissions information, clinical assessments and authorization paperwork of individuals in various identified populations. The IC will assure that all referrals are documented in appropriate database. The IC will coordinate all referral documentation and meet state, agency, and Medicaid requirements. The IC will submit new authorizations/reauthorizations for identified services.

Essential Functions:

  1. Have knowledge of LSC Policies and Procedures Manual, Foster Care manuals, Medicaid Documentation, and authorization procedures specific to the region. Maintain State and Federal standards for service provision and adhere to Council on Accreditation standards (or appropriate accreditation agency).
  2. Engage referral agencies and stakeholders regarding service availability within LSC programs.
  3. Serve as a point of contact for referral agencies and intake documentation.
  4. Identify service opportunities for prospective clients through review of referral documentation and contact with DSS placement coordinators and other referral agency personnel.
  5. Responsible for set up and maintenance of a referral tracking system within the existing LSC database.
  6. Assist with the collection of required documents for authorization and intake. Provide consistent communication to the responsible programs regarding referrals.
  7. Ensure documentation for intake is accurate and deadlines are met for admission.
  8. Provide ongoing coordination with identified programs on appropriate service levels and reauthorization as needed.
  9. Assist with in-service trainings and external reporting as needed.
  10. Represent Lutheran Services Carolinas in the local community. Maintain positive contacts with other service providers for support of programs and advocacy for clients.
  11. Other duties as requested by supervisor
Qualifications

Education: BA/BS degree from an accredited university or college.

Experience: 2 years of experience with service authorization process and documentation and working knowledge of service definitions for foster care services, preferred.

Specific skills/abilities: Familiarity with computer programs for database data entry, proficiency with normal office equipment such as fax, scanner, and copier machines.

Specialized knowledge, licenses, etc: Working knowledge of service definitions for children’s Medicaid and other services. Must be at least 21 years of age and have a valid driver’s license.
