Holon Health’s initiatives develop, support, and maintain relationships with provider partners and community-based organizations to promote programs advocating long-term health and wellness for this complicated population. By acting as the first point of contact for the justice systems and programs with whom we partner, Holon Health provides timely and efficient brief interventions to better prepare patients to receive community-based services and engage in proactive, prosocial behaviors.


POSITION SUMMARY:

Holon Health is seeking an experienced Medical Provider to join our care team. As a Medical Provider you will facilitate care for individuals with Substance Use Disorders (SUD) in collaboration with Care Navigators, Engagement staff, judicial points of contact, and local community-based organizations. Your support is essential to helping our participants find their way through their recovery journey and will allow them to access whole-patient care.


This role is ideal for an individual who has outstanding interpersonal skills and a passion for helping others.


SCHEDULE:

This position can be either full-time (40 hours per week) OR part-time (20 hours per week).


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Assessments & Evaluations: conduct evaluations to assess patients’ status in terms of addiction recovery and overall mental and physical health. Determine medical history, symptoms, and current functioning.
  • Diagnosis & Treatment: formulate diagnosis based on assessment findings and develop individualized treatment plans.
  • Collaboration & Consultation: collaborate with Holon Health’s care team, external medical professionals, and judicial points of contact to provide whole-patient care and coordinate treatment plans. Consult with other NPs and Medical Directors for complex cases.
  • Medication Management: prescribe and monitor medications, ensuring appropriate dosage, potential side effects, and interactions with other medications. Regularly assess patients’ response to medication and make adjustments as needed.
  • Patient Education: Educate patients and their families about health conditions, treatment options, medication management, and self-care strategies to promote overall wellbeing.
  • Documentation & Recordkeeping: Maintain accurate and up-to-date medical records, patient assessments, treatment plans, progress notes, and any changes in treatment.
  • Adherence to Ethical & Legal Standards: comply with ethical guidelines and legal regulations related to mental health and addiction recovery practices, patient confidentiality, and informed consent.
  • Training & Professional Development: actively participate in internal company trainings/meetings and external continuing education opportunities.
Requirements

QUALIFICATIONS:

  • Active and unrestricted Washington license as a Nurse Practitioner, Physician Assistant, or Medical Doctor/Doctor of Osteopathy (additional state licenses a plus)
  • Active DEA in states in which you hold a license; CDS in states where applicable
  • PMHNP/PMH-APRN preferred
  • Several years of experience in the practice of addiction medicine or psychiatry
  • Board certification in area of specialty preferred
  • Strong affinity with and compassion for people with SUD
  • Excellent interpersonal skills
