Description

Aurora Mental Health & Recovery (AMHR) is seeking a Hospital Liaison to facilitate communication between our clinical teams and external psychiatric inpatient treatment facilities, including Crisis Stabilization Units (CSUs), Acute Treatment Units (ATUs), and psychiatric hospitals. The Hospital Liaison will coordinate referrals, unite clients with appropriate AMHR treatment teams, set up appointments, and obtain clinical referral data for the treatment team. This role may also involve ongoing communication with Colorado Access. If you have a hospital background as well as have a solid comprehension of mental illness and treatment systems please apply today!

 

Term-limited: This is a full-time, benefits-eligible position anticipated to last approximately 1 year. If funding is extended, the position may continue beyond this period. Additionally, at the conclusion of the term-limited position, there may be opportunities for the incumbent to transfer to a non-term-limited position, contingent upon meeting the necessary qualifications. Please note that employment is at-will, meaning it can be terminated by either party at any time, with or without cause or notice.

 

Shift. Monday - Friday 8:00 am - 5:00 pm. We have a hybrid work schedule where you can enjoy working remotely half-time and work in the office 2 days per week!

 

Salary for this role. Starting wage is based on experience and company equity. Paid bi-weekly.

  • $19.65 - $29.46 per hour

 

Bilingual Language Differential Pay. *rate depends on language requirements for the position and candidate's fluency.

  • Will receive scheduled earnings on a bi-weekly basis for all hours worked (excluding non-productive hours paid but not worked, e.g., paid time off, holiday, administrative, etc.) in an amount equivalent to 5% of their regular base rate (hourly or salaried), to a maximum amount of $3,000.00 per calendar year. *the annual maximum amount will be prorated based on full time equivalency (FTE).

 

About the team.

The Hospital Liaison will work under the supervision of the adult intensive services division. Teams within this division collaborate closely, providing high levels of peer support and cohesiveness among staff in delivering care. The Hospital Liaison will also coordinate with most clinical teams within the agency.

 

About you.

The ideal candidate will have a hospital background and a solid comprehension of mental illness and treatment systems. They will demonstrate the ability to respond effectively to various client presentations and crisis situations and document interactions with clients, agencies, and mental health professionals efficiently and timely.

 

Essential Functions:

  • Receiving communication from referring treatment agencies via phone, email or fax.
  • Responding to referring individuals and agencies in a timely fashion, usually within the same day, to discuss referral needs.
  • Providing a level of care assessment and determination for purposes of uniting clients with the treatment team that will provide the level of care needed upon discharge from the external treating facility.
  • Monitoring the e-fax for clinical referral data, uploading the data to the clients electronic record, and forwarding the clinical referral data to the assigned treatment team.
  • Maintain a data tracking record with the referral information to include the day of referral and scheduled follow up appointment.
  • Working with assigned hospitals to coordinate referrals of clients from acute psychiatric treatment units to other facilities for more intensive and longer term services.
  • Interacting with the Colorado Access liaisons to coordinate treatment and meet the 7 days post hospital follow up engagement goal.
  • Maintain regular communication with Aurora MHC treatment teams in order to be fully aware of the internal referral processes used to set clients up for their needed services.
  • On occasion provide education to internal and external referring individuals to assist for a smooth and collaborative referral process.
  • Work closely with primary supervisor to problem solve referral issues as they occur. This needs to occur as close to possible in real time in order to expedite the successful referral to post-hospital and ATU services.
  • Coordinate the referral process for certified clients including assisting with setting up the paperwork referral components, scheduling doctor to doctor consultations for referral of certified clients, and working with the intensive services teams to coordinate the team assignment.
  • Some travel within the community may be required. Applicant must have reliable transportation. 
Qualifications

Requirements:

  • Bachelors degree
  • Preferred: Masters degree in a clinical human services field
  • No experience needed. Strong interest in working in Mental Health arena.
  • Ability to comprehend and effectively respond to all client presentations and crisis situations.
  • Ability to document interactions with clients, other agencies and mental health professionals.
